1. This document outlines the protocol for using intravenous glyceryl trinitrate (IV GTN), including starting doses of 0.6 mg/hour for unstable angina or 1.2 mg/hour for left ventricular failure and increasing in increments of 0.3-0.6 ml/hour up to a maximum of 12 ml/hour while maintaining a systolic blood pressure over 90 mmHg and monitoring for symptoms.
2. The protocol instructs to reduce the rate by 0.3-0.6 ml/hour if pain or symptoms reoccur and discontinue IV GTN according to doctor's instructions, such as after 24 hours of being symptom-free.

1. Establish reason for nitrates in order to effectively evaluate desired outcome.
2. Establish base line observations of :- BP (at least 90 mmHg systolic)
Pulse Respirations & breathing pattern
3. Commence infusion at * 0.6 mgs/hour (ie 0.6 mls/hour of 50 mg in 50 ml concentration)
** 1.2 mgs/hour (ie 1.2 mls/hour of 50 mg in 50 ml concentration)
* Dose for unstable angina
** Dose for LVF
Increase GTN only in steps of 0.3 ml - 0.6 ml per hour
BP >90 mmHg systolic at approx 30 minute intervals. Patient pain free (unstable angina) No Check BP every 15 minutes on drug increase. Breathing comfortable (LVF) Maintain 90 mmHg systolic BP.
Yes Symptoms resolving No Increase to a maximum of
12 mls per hour in increments of Maintain at present dose. 0.3 - 0.6 mls per hour at approx 30 minute intervals. Monitor BP every 15 minutes BP every 15 minutes maintaining for first hour & then at least Yes 90 mmHg systolic. every 4 hours as required. Pain/symptoms reoccur
Reduce rate by 0.3 - 0.6 mls per hour. Symptoms
Check BP hourly or as per resolving individual requirements. Maintain at 0.6 ml per hour if Yes symptoms resolved
