Good day!

I am writing this letter to formally request from your office certified true copies of
the following documents:

1. Two (2) certified true copies of the Resolution dated September 22,
1993 for the case entitled Maria _________ Vda. Del Rosario, et. al. vs.
Violeta Del Rosario Jao, et.al docketed under G.R. No. 111150 and:
2. Two (2) certified true copies of the Entry of Judgment issued for the
case entitled Maria _________ Vda. Del Rosario, et. al. vs. Violeta Del
Rosario Jao, et.al docketed under G.R. No. 111150;

The request is made on behalf of my client, JESSICA DEL ROSARIO JAO of

Barangay Cabaruan, Cauayan City, Isabela, who is a legal heir of Violeta Del
Rosario Jao, one of the respondents in the case above-mentioned. The
requested documents will be utilized for further legal actions and/or proceedings
which my client seek to pursue regarding the case.

As per instruction, attached in this letter are the following documents for your
ready reference.

1. Certificate of Live Birth of Jessica Del Rosario Jao issued by the

Philippine Statistics Authority;
2. Duly signed photocopied of her driver’s license and UMID identification
card;
3. Post money order amounting to five hundred forty pesos (P540.00) with
check number BDO-0100018119 dated July 16, 2020 payable to the
Supreme Court of the Philippines and
4. Eight (8) pieces documentary stamps (brown color worth P15.00 each)
