 The PRF should be submitted in a Portable Document Format (PDF), accompanied by your latest evaluation
sheets, and emailed in a single file only, with a file name and an email subject following this format:
YEAR – LAST NAME, FIRST NAME M.I.
(e,g, THIRD YEAR – JUAN, JUANA J.)

 Evaluation sheets are downloadable via AKAN accounts or you may secure the same from MSU CIT. Should you
not be able to secure yours, you may still send your PRF but make sure to bring your printed evaluation sheets
during the enrollment period.

 Submit the duly accomplished PRF and your evaluation sheets to lawstudentgovernmentmsu@gmail.com not
later than July 28, 2020.
