Tech. Superintendent (T.S.) to list down all Deck Deficiencies for monitoring. The form Non-
Conformity Report (NCR) and Deficiency/Defect Report (D/R) for Deck Deficiencies should be
attached immediately after this page.
Master should revert by Mail/Fax on monthly basis to TS and DPA, the work progress. In their message,
the Prefix D01…D10 should be used to indicate the respective items.
D) After the Master's final report, T.S. to close out this part of the Vessel Inspection Report with
the following:

Conformity Report (NCR) and Deficiency/Defect Report (D/R) for Engine Deficiencies should be
attached immediately after this page.
The Master in consultation with Chief Engineer should revert by Mail/Fax on monthly basis to
TS and DPA, the work progress. In their message, the Prefix E01…E10 should be used to
indicate the respective items.
D) After the Chief Engineer's Final Report, T.S. to close out this part of the Vessel Inspection Report with
the following :
