A Narrative Report

I. Introduction

In this part, write a brief introduction about the activity. Include the important details

like the date, venue, and purpose/objectives.

II. Activity Proper

In this part, write/type the sequence of the events happened during the activity.

Highlight those parts that are relevant to the program objectives.

III. Significant Accomplishments

Here in this portion, outline the significant accomplishments or achievements of the

activity. This section prefers number or figures as accomplishment. You may use

table for organization, supported by verbal description. Examples are:

1. You may write here the number of attendees over the expected participants.

2. You may also include here the evaluation results if available or there are.

3. You may also share here the number of activities/parts conducted.

IV. Issues and Challenges

In this portion, do not forget to mention the problems you or the participants have

encountered in the pre-implementation, implementation, and post-implementation of

the activity.
V. Recommendations

Consider this part as your way of providing insights on how can the activity be further

improved in the next implementation. If you believe that the program has been very

successful, you may instead write commendations on the best practices you have

observed.

VI. Reflection

This section reveals your learning and realization about the activity. It mainly

answers on how do you think this activity can generate learning and improve the

school system and/or processes.

VII. Attachments

In this section, put some of the means of verifications. You may include some

pictures with captions, attendance sheets, and others.
