AFFIDAVIT OF UNDERTAKING

I, PLARIDEL F. ADIWANG, of legal age, married, Filipino, and resident of

Santo Tomas, Km 7, Baguio City after having been duly sworn to in accordance with
law, do hereby depose and state THAT:

1. I am the lawful owner of a certain Land Cruiser motor vehicle covered and
embraced by Certificate of Registration (CR No.) 19777285 with Plate No.
XGA332, Engine No. 1HZ0009128 and MV file No. 0386-00000029783 by
virtue of an Open Deed of Sale signed and executed by its original owner,
Thomas Limsico;

2. However, in the Certificate of Registration, no color was indicated therein.

Nevertheless, the said vehicle is colored Gray;

3. The omission of the foregoing was probably due to oversight and not intended
to mislead nor defraud any individual, public or government authorities;

4. I hereby undertake to fully excuse the government for whatever damages or

liabilities it may incur arising for the indication of the color of my said vehicle
in the Certificate of Registration (CR);

5. Finally, I am executing this affidavit to attest to the truthfulness of the

foregoing facts and for all legal intents and purposes it may serve well.
