AFFIDAVIT OF CHANGE OF ENGINE

I, REY D. PAVILLAR, Filipino, of legal age, married, and a resident of 18

Gwaping St. Block 5, Ubas Compound, Phase 1, SIR New Matina, Davao City, after
having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the registered owner of a Suzuki Jeepney with plate no.

LWJ-266, make SUZUKI, serial/chassis no. DB71T-328868 and
motor no. 2634261;

2. That the above-mentioned motor /engine is already changed ;

3. That new motor/engine no. is FGA-5702640;

4. That I am executing this affidavit to apprise the LTO and the

authorities concerned on the veracity of the foregoing facts and
circumstances and for whatever legal purpose this may serve.
