    AFFIDAVIT

    I, Mrs. Priyanka Pravin Jain having address at E- 804, Teakwood , Vasant

    garden, Mulund west, Mumbai-400080 do hereby solemn affirmation as under:

    1. I state that there is no criminal and antecedent registered against me

    with any Police Station.

    2. In case, if any offence is being found to be registered against me, in any

    Police station, result into conviction, I will surrender my sanad issued by

    the Bar Council.

    3. That by virtue of this affidavit, I changed my name from Miss Priyanka

    Nemchand Porwal to Mrs. Priyanka Pravin Jain and henceforth I shall be

    known as Mrs. Priyanka Pravin Jain for all purposes.(Copy of gazette

    showing name change attached)

    That the facts stated above are true to the best of my knowledge and belief.
