This Deed of Donation made and executed by:

Lot 16 Ph 2, Amparos Village, Villareal, Bayawan City, Negros Oriental, herein referred
to as DONOR;

EVA M. TRAYVILLA, Filipino, of legal age, widow, and a resident of 468

Magasaysay St., Brgy. Boyco, Bayawan City, Negros Oriental, herein referred to as
DONEE;

WITNESSETH:

That the DONOR is the owner and plan holder of a St. Peter Life Plan Inc.,
Memorial Plan known as St. Hyacinth valued at Php 15,000.00 and was used for the
funeral and burial services of the DONEE’s husband, ROBERTO F. TRAYVILLA;

That for and in consideration of the love and affection which the DONOR has for
the DONEE, the DONOR hereby TRANSFERS, CEDES and CONVEYS ABSOLUTELY
by way of DONATION unto JINKY, her heirs, executors or administrators, and assigns,
all the rights, interests and participation of the DONOR in the St. Peter Life Plan Inc.
Memorial Plan known as St. Hyacinth valued at Php 15,000.00.
