AFFIDAVIT OF LOSS

I, MARIA EDLYN ESPIRITU, of legal age, Filipino with postal address at Libra St.,
Polomolok, South Cotabato, after having been duly sworn to in accordance with law, do hereby
depose and say:

1. That sometime on May 12, 2012, while I was riding on a jeep going to DARBCI, my
wallet fell off without my knowledge;

2. That my wallet contains the following important documents, to wit:

a) SSS ID
b) CARITAS ID
c) DRIVER’S LICENSE
d) PHILHEALTH ID
e) KCC CARD
f) GAISANO CARD
g) CLAIM SLIP FOR PRC ID

3. That diligent efforts exerted to locate my wallet but it proved futile as the same
could no longer be found or located;
4. That my said driver’s license was not revoked , confiscated, or being withheld by
lawful authorities for violation of any law, rules or regulations ;
5. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever purpose this may legally serve.

1. That I am a holder of Plane Ticket route: Manila-Kuala Lumpur-Manila, which I

purchased through Marazul Travel, Inc.-Marisa Bautista;
2. That the just recently, when I looked over for said plane ticket, the same could not be
found;
3. That diligent efforts were exerted to locate my said plane ticket, proved futile as the same
could no longer be found or located;
4. That I am executing this affidavit to attest to the truth of the foregoing facts and for
whatever purpose this may legally serve.
