With reference to the subject, I regret to inform you that your performance has not been up to
the standards demanded of Staff and Twice, you have orally warned about your lack of
concentration, coming late to duty and carelessness.
In Particular,
On 13th September,2011, a subscriber booked a compliant in the compliant register
which is available at Our Office Gate regarding his telephone fault ( Telephone No.238546 ).
Immediately it was informed to Test-Desk by security personnel. On that day you were on duty as
per the duty chart and you had received the call, but you didn’t do the testing for the forementioned
number.
Due to your carelessness, unnecessarily the service had been interrupted for the last 7
days.
You are here by called for an explanation in this regard. The explanation should reach
the undersigned within 3 days.
