What is a Medicine Use Review (MUR)?

A Medicine Use Review Assessment is a FREE one-on-one consultation with a qualified community
pharmacist for patients with:
 medicines adherence issues
 poor understanding of their medicines
 or confusion with their medicines.
The MUR consultation involves:
 An MUR pharmacist meeting with an eligible patient in pharmacy or at the patients own home
 Collecting information about patients health, their history, and their medicines.
 Offering medicine advice and solutions identified in partnership with the patient
 Reporting to the patients GP with issues identified, solutions offered and adherence
recommendations made
What is the Patient Eligibility Criteria?
The MUR service is eligible to people:
 Living independently in the community with a chronic condition, plus one or more of the
following:
 Takes 3+ medicines and/or 12+ doses per day
 Has multiple prescribers
 Had a recent admission to hospital (especially if there had been a medicine change)
 Is taking medicine(s) with a high risk of adverse effects
 Is experiencing sub-optimal response to medication
 Is non-adherent or unable to manage their medicines
 Has literacy or language difficulties, dexterity problems, impaired sight, or cognitive deficiencies
which impact on their ability to manage their medicines
 Is taking a narrow therapeutic index medicine eg warfarin, amiodarone

Exclusion criteria for the service is the patient can’t be living in assisted care (eg. rest homes) – if they are
living in an independent villa within the retirement village we can visit them. Also patients have to be living
in the Waikato region.

 Advising when, how and what is the best way to take medicines

Who can refer a patient to this service?


 Health providers e.g. GPs, Practice Nurses, District Nurses, Diabetes Nurses
 Pharmacists
 Patients
 Family/Whanau and Caregivers

How to refer:
I’ve attached our referral form. Ideally, our referral form should be emailed
to pharmacyreferrals@midcpg.co.nz – this is a mailbox that both mobile pharmacists can access and
ensures that if one of us is away, the other can pick up on referrals.

However, we’re very flexible in how we receive the referrals – some choose to ring, fax or email us (with or
without the forms). Whichever is the easiest method for you is fine with us.

If you have access to the BPAC e-referrals – there is an option to send a referral to Mobile Clinical
Pharmacist.
