This form may be submitted by a member who wishes to relinquish the ownership of his / her / their
current position with [user name]. By submitting this request, the member acknowledges that he /she /
they will no longer be the owner of this account and are transferring ownership to an individual who is
not currently enrolled. If this account is the member’s only account, he / she / they will no longer be a
member. Information with an asterisk (*) is required. Incomplete forms will not be processed until all infor-
