The meeting was called to order at 2:00 pm and the following matters were discussed and agreed upon:

Objectives:

 Removal of Tempacil specially contractors without project


 Pull out and remove all Garbage and materials not needed in the area
 Re – updating of portable electrical equipment.
 Sustain proper housekeeping, improve 5’s plan

Based on Observation inspected by Project team at Contractors Temporary Facilities on November 6,


2017

Findings as follows:

 Not Conducting Housekeeping inside Staging area


 Improper storing of Painting materials inside staging area (no catch pan)
 Storing of Hygiene Suite inside Temfacil
 Storing of Excess and scrap materials inside and outside temfacil
 5’s plan not followed
 No proper frame and handle installed on door of the temfacil, all sharp edges must be protected
 Standard size of caution paint not followed (Zebra Paint)
 Not following standard Height of Temfacil
 Unsafe condition of staging area
 Unacceptable use of corrugated sheets as temfacil
 Improper storing of PPE’s
 Un -attended materials and equipment has been observe on working area while breaktime
 Sitting on the Gutter while Conducting Toolbox Meeting
 Project Contractors without P.O must remove temfacil and pull out all equipment and materials

Recommendations & Instruction

 Conduct Housekeeping in staging area everyday


 Provide catch pan on used painting materials, including properly disposal of excess painting
materials must follow
 Provide Plastic box for storing of hygiene uniforms and must place inside the container locker
room, provide proper label
 Pull out materials not need in the area, scrap materials must place in scrap yard
 Improve temfacil and make sure that there are no sharp edges
 Standard of Caution Paint (Zebra Paint) must be observed and follow:
o Zebra Paint must 300mm Depth
o 200mm Yellow and Block Slant Thickness
 Follow the standard Height of Temfacil which is 2.4 meters
 Clean the staging area before and after work and must have pad lock
 All temfacil must be plain sheet with metal furring as frame
 Proper storing of all PPE’s must be observed and place in one designated area
 Dismantled fixtures equipments which is NPI Property, (Ex. Lighting fixtures, Tempered Door,
Aircon, etc,) must be turn over to Project Store, Look for Mr. Condrado Colcol TBMC for proper
storage

Note: Agreed completion of discussed findings is on or before Nov. 8, 2017

There being no other matters to discuss, the meeting was adjourned at 3:00 P.M.
