Suggested Laboratory Specific Biosafety Manual Format

Laboratories using biohazardous agents must have a biosafety manual available for immediate
reference. Listed below are some suggestions for information that could be included in this
manual.

Section 1. Emergency Response Plan to include the Primary Investigator’s contact


information, as well as the contact information listed below where
applicable.

Fire Ext. 9-5555
Medical Emergency Ext. 9-5555 (pg 888-OUCH)

Section 2. Copies of all IBC, IRB, and/or IACUC protocol(s) with approval letters.

Section 3. Abbreviated Standard Operating Procedures for each IBC-approved protocol.


(Specialized laboratory SOPs for each protocol should be available within the
laboratory. A simplified template for compiling the SOP is attached.)

Section 4.. Laboratory Inspection Checklist and approval letter.

Section 5. Copy of each participating individual’s Biosafety Training Certificate of


Completion.

Section 6.. Copies of relevant reference materials


(This may include materials such as the UNMC Biosafety Manual [available at
www.unmc.edu/ibc/], pertinent sections of the NIH Guidelines, and/or the
CDC/NIH BMBL Manual. Web site addresses for important
information pertaining to the laboratory may also be included.)
