PROJECT
Each person on the site is obliged to follow the pertaining rules. It is the Suppliers’ responsibility to inform their
employees about these regulations. Failure to comply with these regulations may lead to removal from the site.

1. GENERAL

1.1 INTRODUCTION
This safety manual contains the minimum set of safety rules and procedures. In addition, all safety rules
and procedures of the local and central Government Authorities of Viet Nam shall be adhered to.
Safety rules and procedures are an integral part of each job and must be carried out fully. Any exception
to the rules and procedures, during the course of any job, must have prior written authorisation.
Staff and workers of Contractors who exhibit a poor attitude toward safe work and safety procedures will
not be acceptable. Employees leaving this job due to safety violations will not be allowed to return to
work without proper authorisation.
Each contractor, civil consultant & VBL Project team, in the role of Consultant Representative for site
supervision and project management, will take and active role in ensuring that the Contractors will be
operating in accordance with strict safety procedures.

1.2 SCOPE
These regulations apply to Contractors working at the construction site of VBL Da Nang at Da Nang
City/ Viet Nam.
The Contractors and their personnel, including all Sub-Contractors and their personnel, shall comply
with the rules and regulations of the Installation SHE Plan including applying Site Regulations.
Each Contractor shall be responsible for the safety of his own staff workers and Sub-Contractors staff
and workers on the Works and should have its own specialised Safety Rules to carry out Contractor
specialised works.
Each Contractor shall appoint a Contractor’s Representative. Ditto shall submit personnel details of a
senior member of his supervisory staff who is nominated as the Contractor's Safety Officer.

1.3 DEFINITIONS
Site: A production facility that is identified by physical, geographical or logical segmentation.
Installation: An event on Site to realise an Investment Project.
Contractor: Suppliers of Plant & Machinery and adhering provisions for the Installation.
Consultant: External Consultant – Main Contractor – for Civil Works.
Project Responsible: HSC responsible /representative for the Project including Installation.
Installation Manager: Responsible for the Installation regarding the daily operations on Site.
Resident Engineer: The engineer that monitors the Civil Works in Investment Projects on
behalf of the Project Responsible.
Site Representative: The person(s) authorised to represent the Site for a certain subject
(Safety /Security Officer, Chief Engineer, Brewery Manager, Training
Manager).
Installation Representative: The Overall Responsible on Site during the Installation. The Installation
Manager, Resident Engineer and Project Responsible in descending order
shall be the designated Installation Representatives.
Contractor’s Representative: The person authorised in writing by a Contractor to be responsible for the
work on a full-time basis, and who can be contacted at any time.
Consultant’s Representative: Ditto for Main Contractor – the Civil Consultant.

2. SHE LEGISLATION

2.1 LABOUR LEGISLATION


Depending on the local situation legal conditions for labor circumstances applies to the installation,
commissioning, operation and maintenance of Plant and Machinery.
The ‘Arbeidsomstandighedenwet (Arbowet)’ describes the right HSC and duties for employers and
employees concerning their responsibilities for Safety, Health and Environmental affairs on the job. This
law or equivalent local law adheres to the installation job.
In cases local legislation is not available or is not meeting the minimum Concern requirements on SHE,
the Arbowet is put in place as reference for the Installation /SHE Plans. This subject has to be checked
prior to the arrangement of the Plans.

2.2 MACHINE LEGISLATION


Depending on the local situation legal conditions for Safety, Health and Environment applies to Plant and
Machinery; development of equipment and the construction of production facilities.
The CE (Conformité Européenne) Marking describes the requirements on Safety, Health and
Environment for machine or installation. This legal directive adheres to the Installation Job for countries
within the EEA (European Economic Area).
In cases local legislation is not available or is not meeting the minimum Concern Requirements on
Machine Legislation, the standard pr NEN EN 1672-1 (CE Marking) can be put in place as reference to
the Plans for Installation and SHE. (Check CE Marking requirements in advance).

2.3 SAFETY NORMATIVES


ISO 3864 and NEN 3011 serve as standards to Safety Signaling.
To various European countries the EEG directive 92/58 applies as law (to be checked).
HMESC 879803 is a corresponding Heineken standard for Pipe Markers.
ISO 5579 applies to radiographic examination of metallic materials by X- and gamma rays.
It is legally obliged to be aware of the risks that go along with the usage of a certain product. To that per
product a standard Transport Emergency Card (TEC) is required with up-to-date safety information. For
products for which TECs do not exist, Contractors shall take care of comparable information sheets.
The Safety Officer should maintain a register of chemicals (type, location and quantity).

Appendix A reflects an overview of Safety Signs deriving from ISO 3864, see also paragraph 7.10
for additional instructions on the usage of chemicals.

2.4 MINIMUM SAFETY REQUIREMENTS FOR CONTRACTOR


 Have a thorough, well written and implemented Safety Policy which includes a statement addressing
drug and alcohol abuse;
 Commit a Contractor’s Responsible for, and active in, day-to-day safety activities on the job site;
 Have a self-auditing program for inspecting, correcting and assuring safe work conditions and work
practices on the job site;
 Have an ongoing Safety Program which includes new employee safety training, regular safety
meetings and reviews of safety activities and incidents;

 Assure that all staff and workers are knowledgeable of, and in compliance with the "Contractor's
Safety Rules", and all applicable labour law of the labour department of Viet Nam;
 Have a procedure to assure that (Sub-)Contractor employees review /understand job site safety
information and requirements (safety induction during the first day of work on site, see 5.2).
 Exclude the usage /application of products like: Asbestos and Halons (check on more!!)
3. SITE ACCESS AND CONTROL

3.1 THE BREWERY SITE


The Brewery is being expanded on a site of approx. 290m x 240m. A fence (height approx. 2 mtr.)
surrounds the complete site and there is one main entrance. Also two guardhouses have been installed for
security reason. The Brewery Site will be secured by guards, 24 hours per day, 7 days a week.
3.2 SITE ACCESS
The only entrance is located at north east of the Site.
There is a barrier and a guardhouse behind the gate. This barrier is closed (= down) all of the time. It can
only be opened shortly to give access to an authorized vehicle. It is closed immediately afterwards.
Between the barrier and the guardhouse persons who are walking can enter the brewery site.
Only authorised people and vehicles – in possession of necessary autorisation - see paragraphs 4.3 and
4.4 - are allowed to access the Site. To notify all persons about this, a big signboard (1  2 Mt.) is located
at the gate with the text (in both English and in local language):

This means that everybody should register themselves (and their vehicle if applicable) to obtain a permit
at the guardhouse which is located at the main gate. Group transportation like vans and trucks are not
allowed to enter with persons inside. Persons should get off and get in/outside the gate and walk.
All employees/workers/vehicles entering or leaving the site may be subjected to inspection by authorised
guards or personnel of the Site.

3.3 SITE CONTROL


Each Contractor is responsible for the people working on the site under his supervision and has to give
full co-operation with the identification system for them. The system consists of a register of workers
names kept on Site by the Installation Representative. Each person receives a Workers Pass with
photograph.
Before starting the work, each Contractor shall take care of /indicate:
- Submission of two photographs of each staff member and worker for the identification system.
- Names and I.D. pass of personnel with their respective trades classification on the Site;
- Name and driving license of driver and registration number of vehicles regular accessing the Site;
- Any special equipment requirements, e.g. for cranes, mobile compressors, etc.
Contractor staff and workers will confirm themselves to the work site and specified access routes and
will not enter any other plant, utility and administration area, workshop or amenity area.
(Sub-)Contractors shall instruct their Vendors to deliver materials and equipment to the Contractor's
office on the job site. Deliveries shall not be made directly to the Site unless the Contractor's
Representative has arranged to meet the delivery. The Installation Representative must be given a list of
Vendors in advance so that he can make arrangements with the Security Officer. Persons who deliver
materials and equipment shall not wander about the site looking for a Contractor (Representative).
To Site Access and Control the various types of passes exist i.e. Workers Passes, Visitors Passes, Car
Passes, Office Passes and there can be others like e.g. Equipment Gate Passes, see paragraph 4.4.

The Security Officers are entitled to check at any time all persons, bags and vehicles before entering or
leaving the Site and also during their staying on the Site.

3.4 AUTHORISATION PASSES


All passes are to be worn and shown clearly during the stay on the Site and must be presented for
security check. The passes are Site property and must be returned upon termination or upon request by
the issuing authority. Lost cards should be reported to the administration office immediately.

1 Workers Passes
See for the obtaining of a Workers Pass at paragraph 3.2. At the end of each day this pass should be
handed-in at the guardhouse when leaving the site. The next day the same pass has to be collected again
by informing the guard about your personnel number.

2 Visitors Passes
Visitors for the site must obtain a “Visitors pass”. This procedure must be followed for every visit. When
they register their personal name, company name, identification card number /business number /card and
purpose of the visit, they will receive a temporaryVisitors Pass with a unique number.
The visitors must present themselves immediately to the Installation Representative. The pass shall be
handed-in at the guardhouse when leaving the Site. The Officers register date, arrival and departure time.

3 Car Passes
In principle cars /trucks should be parked outside the Site. But in case a vehicle needs to enter, a “car
pass” must be obtained before entering. When the numberplate, name of owner/company has been
registered, the Officer will handout a Car Pass with a unique number. The pass should be positioned
behind the front window screen (right hand side) to be seen clearly. The Car Pass has to be handed-in at
the guardhouse when leaving the site. The Officers register date, arrival and departure time.
For cars which should enter the Site regularly, like for VBL Da Nang and Installation Manager, this car
pass will be personal / permanent and therefore can remain in the car when leaving the Site. Only the
Installation Representative is authorised to decide which cars will get a permanent pass.

4 Office Passes
Only the Site Office Personnel who wear an official Site Office Pass with photograph and who also have
a permanent Car Pass are allowed in without registration. Other Site Office Personnel should apply for a
Visitors Pass (see item 2). If they need to enter by car they should also obtain for a Car Pass (see item 3).

5 Equipment Gate Pass


Removal of Contractor's material /equipment must be covered by a "Contractor's Equipment Gate Pass".
If special items are brought in, which will be taken out of the site after the visit, the items should be
registered at the guardhouse. The Officer issues and signs an authorisation paper (“Permit for temporary
bringing of materials”) to bring in and to carry out the items as mentioned. When leaving the Site, this
paper must be handed-in to the Officer again. The Officer signs this paper for approval when OK.
A similar paper (“Permit to retrieve materials from VBL Da Nang Site”) is used for all materials that
will be removed from Site. Only the Contractor’s Representative can issue this permit. Permission for
removal should be applied for in advance and can only be authorised by the Installation Representative
/replacer.

3.5 SECURITY OF INSTALLATION MATERIALS AND EQUIPMENT


The incoming materials, which cannot immediately be installed or stored into closed buildings, will be
stored temporarily in special dedicated areas. Due to installation and layout technical reasons, they
cannot be concentrated in 1 (one) central area and therefore not be secured for 100 %.
Although VBL Da Nang /HSC have arranged a certain level of security, the Contractors /Owners of the
materials will be responsible for their own equipment and material during the complete installation
period. No claims for damaged or missing material can be accepted by VBL Da Nang /HSC.
4. SITE REGULATIONS

4.1 REQUIREMENTS FOR STAFF AND WORKERS


It is the Contractor's responsibility to ensure that all his staff and workers are in good physical condition,
i.e. have adequate hearing /sight, possess all limbs, do not suffer from vertigo, etc. The Installation
Representative will have the final say in any problem arising in this area.
Where the plant's safety aspects require, the Contractor shall notify Installation Representative of colour
blindness and the wearing of contact lenses by workers prior to working.
It is essential that all key employees on site can speak, read and write either Viet Nam or English. Where
required by contract or by job instructions, the Contractor shall supply employees who can speak, read
and write reasonable English.

4.2 VEHICLES AND EQUIPMENT


Employees will comply with the governmental safety /traffic rules unless different indicated below.
No traffics allowed on site without the permission of the Installation Representative. Permits to enter the
site are given on name of the company and registration number of the car.
Maximum speed on the site: 10 km/h. Parking on the site only in approved designated areas.
Employees in vehicles must be sitting down at all times when the vehicle is in motion and must not have
any part of their body over the sides.
Employees are not allowed to be transported by vehicles not designed for personal transport (like Forklift
trucks, Shovels etc.).
Contractors shall provide vehicles and equipment, which are in good condition. Vehicles used for
transporting people must have adequate safety provisions to prevent injuries from falls or sudden stops.
Keep all roadways; access doors and fire hydrants clear. Notify the Installation Representative before
blocking any roadway. There must not be over-crowding.
The number of vehicles, which are allowed daily residence on site, must be kept to an absolute minimum.
Equipment such as welding machines, pumps etc. shall not be transported with the engine running.
All equipment, machinery and tools for use on the job site must be approved by the Installation
Representative and shall be subject to initial and periodic inspection. Any equipment, machinery and
tools, which are not approved, must be removed from the site.
Any equipment, machinery and tools, not be supervised, should be disconnected.
Refuelling equipment while the engine is running is prohibited.

4.3 CONTRACTOR'S RISK
Each Contractor shall provide insurance coverage for loss or damage to his equipment, materials, and
plant and those of his staff, workers, Sub-Contractors and Suppliers as stated in his contract. The
Employer shall not be held responsible for any such loss or damage. Each Contractor shall provide all
insurance coverage for his staff and workers and his vehicles that is required under the laws and
regulations of the local and Viet Nam.
Whenever equipment or tools are lost or stolen, the Contractor should report the details to the Installation
Representative as soon as possible.
4.4 PHOTOGRAPHIC EQUIPMENT AND RADIOS
Photographic equipment (camera, video etc.) is not permitted on Site, unless prior approval in writing
from the Installation Representative. Use of such equipment shall be restricted to the locations and
subjects specified on the approval. Any of such equipment found on Site, not approved for use may be
seized by Installation Representative, who may arrange for any film to be processed and examined. The
equipment will then be returned to the Contractor.
4.5 TIME KEEPING AND JOB CONTROL
When a Contractor wishes to work before or after regular hours, or on Sunday or Public Holiday, he must
have authorisation from the Installation Representative.
Contractor's staff and workers are only permitted to eat or take a rest at the designated areas.

4.6 RADIATION
All governmental laws pertaining to the use, storage or handling of a radiation source must be complied
with. The Installation Representative must be notified prior to any radiological work being commenced.
The necessary distance to radiation source should be taken care of, see ISO 5579.

4.7 PRESSURE TESTING


The necessary distance to the equipment/ piping pressure tested should be taken care of minimal 30 m
safety distance and minimal 10 minutes waiting time.

4.8 ENVIRONMENTAL CONTROL


Each Contractor is responsible for the Environmental Control specified for the job site, including all
equipment and machines used. For that reason designated drums should be available.
Each Contractor shall instruct his staff and workers not to dispose of any used oil or liquid waste direct
to the ground, pit or storm drain; this regulation shall be strictly enforced.

4.9 HYGIENE
The Contractor shall maintain good hygiene at Site, which shall involve.
- Provision of and maintenance of adequate numbers of toilets, washbasins and showers for all
workers; these areas shall be kept clean at all times.
- Provision of adequate canteens for his staff and workers.
- Prevention of unhealthy site conditions, such as puddles that attract mosquitoes, wasteheaps that
attract flies.
- Cleaning of the Site and of buildings and structures on a regular basis, and removal of waste and
debris from the Site.
- Provision of adequate septic tanks for all temporary toilets and regular cleaning of the septic tanks;
these devices shall meet the requirements of the government or as applicable for the region.
- Contractor's staff and workers are not permitted to eat or take a rest at work areas.

4.10 AUDIT FOR CONTRACTOR SAFETY


Installation Representative has the right to conduct Audits of Contractor safety items and conditions, and
all Contractors are obliged and responsible to correct, improve and maintain all safety requested from the
Audit's results.

5. SAFETY PRECAUTIONS

5.1 SITE SAFETY - GENERAL


Each person on Site is obliged to follow the pertaining rules. It is the responsibility of each Contractor to
inform his staff and workers about these regulations and to make available the necessary means for
personal protection and prescribed protective and safety devices and a first aid box. Failure to comply
with these regulations may lead to removal from site.

5.2 SAFETY INDUCTION


Contractor staff and workers must be inducted to these Safety and Security Regulations by the
Contractor's Representative or his deputy during the first day of work on Site.
The main Civil Works Contractor shall arrange a safety meeting before fieldwork is started. Attendance
shall include all Sub-Contractors plus a Site Representative.

5.3 PERSONAL PROTECTION


Each Contractor must ensure that his staff and workers are provided with suitable clothing, safety
helmets, footwear, and if so required goggles or gloves. Sturdy shoes are the minimum requirement;
safety shoes are recommended. Open toe shoes shall not be worn. No bare feet or thongs are allowed.

Each employee shall recognise the use and importance of special protective equipment and clothing.

5.4 EYE/EAR PROTECTION


Safety glasses, complete with side shields (or mono goggles), will be required to be worn by workers
engaged in activities that can cause mechanical eye damage.
Ear plugs/protectors shall be used to protect from noise induced deafness.

5.5 SAFETY HELMETS


Safety helmets are required in places where people are working overhead for all workmen and
supervisors. During the installation period of the brewery equipment, the total construction area is
considered "hard hat" area.

5.6 SAFETY HARNESS /BELT


Required to be worn and tied off when working in dangerous positions or high elevations.
- Steep, sloping roofs.
- Flat, high roofs without handrails within one meter of the edge.
- Generally, at high elevations or above dangerous conditions.

5.7 WORK IN MALT RECEPTION AREA


- A mask shall be worn while working at the malt reception area where malt dust is present.
- Ear plugs/protectors shall be used at the malt reception area to protect from noise induced deafness.

6. INDIVIDUAL ACTION

6.1 ALCOHOLIC DRINKS AND/OR CONTROLLED DRUGS


Alcoholic drinks and/or controlled drugs are not to be taken on site at any time. Anyone found under the
influence of, or in possession of, alcohol or drugs on the site will be immediately removed from the site
and refused future access.

6.2 SMOKING
Smoking is not permitted where any on site except in special designated areas. The Brewery under
special permits MAY declare temporary buildings used by a Contractor safe areas. Smoking in vehicles
on the site is not permitted. Smoking areas will be so designated by Brewery-approved signs.
Smoking shall not be permitted in any building under construction, but a designated location close to the
construction area may be approved.

6.3 MISCELLANEOUS
Horseplay, fighting, gambling in any form is not allowed.
Running at the job site must be avoided except in case of emergency.
Staff and workers must not make inoperative safety devices or operate safety devices such as relief
valves, gate valves, electrical and mechanical interlocks, guards, electrical switches, etc.
Unsafe shortcuts, such as walking on pipelines, sliding down pipes or stanchions, cutting corners through
plants, which are not the work location, must be avoided.
Tools, materials or equipment must not be dropped or thrown from a height to a lower level.
When chemical type goggles are in use, the headband must be around the back of the head, not around
the safety helmet.
Workers with long hair or wearing a necklace are not allowed to work near rotating machinery, workers
hair shall be tied back and held in a net.
Workers who are required to work on electrical systems are not allowed to wear rings watches with metal
band, or metal bracelets.
Hard hats must not be decorated, defaced or damaged in any way.
In addition to any specific requirements noted on permits, the practices from chapter 8 shall be followed:
7. WORKING WITH TOOLS, EQUIPMENT AND MEANS

7.1 TOOLS AND EQUIPMENT.


- Must be in good condition and currently inspected.
- Must be used only by qualified people.
- Tools must be used only for their designated purpose and if unattended to be disconnected.
- Equipment with gears, belts, coupling, fans, etc., shall not be operated without guards.
- Electrical tools and equipment operating at more than 32 volts shall be properly grounded.
- Electrical power distribution boards shall be equipped with Residual Current Circuit Breakers.

- Dedicated gloves must be worn while using grinders, drills, saws or any tools
- Loose clothing must not be worn while working around machines.
- Portable electrical tools shall be subject to inspection by Installation Representative before use.

7.2 ORDERLINESS
Orderliness is a basic requirement and must be maintained at all times. Special attention must be given to
maintaining clear walkways and roadways, removal of trash, removal of slipping and tripping hazards,
and proper storage of materials.
No material or equipment shall be moved onto the site without permission of the Installation
Representative. The only material or equipment that will be allowed in any active work area will be that
material immediately processed into the work.

7.3 PROTECTION OF PROPERTY AND EQUIPMENT


- The Contractors shall at all times provide protection against the weather (rain, wind, storms, dust or
heat) so as to maintain all work, materials, apparatus and fixtures free from injury or damage. At the
end of the day's work all new work likely to be damaged shall be covered.
- Barricades will be needed for excavations, near roof edges, around overhead work and similar areas.
Hole covers or barricades must be placed at all floor openings.
- All holes or openings through floors or decking at all elevations must be provided with hole covers or
barricades /barriers immediately. Do not store material or equipment on a whole cover.

7.4 INDUSTRIAL GASES


All gas cylinders must be handled with care, properly supported in an upright position away from any
source of heat or flame, and securely tied off. Tie-off shall be around the upper body of the cylinder, not
around the fittings. All cylinders not in use must have their caps in place and stored as described above.
Compressed gases must not be used for cleaning of clothes, the body or working areas. Oxygen must not
be used for pressure testing purposes.

7.5 CHEMICALS
- Label all containers or chemicals correctly.
- For Safety Signs, according to ISO 3864, see Appendix A
- Do not take food or drink into places where hazardous chemicals are used or stored.
- Paste warning notices on containers of hazardous chemicals.
- Use personal protection equipment and clothing provided when handling chemicals.
- Observe personal hygiene
- Whenever there is any injury caused by chemicals, seek first aid treatment at once.
An updated register of chemicals should be available at the Safety Officer (or replacer).
7.6 LADDERS
- Straight and extension ladders must be tied-off at the top.
- Stepladders must be fully opened and set on a level surface.
- Work facing the ladder, with both feet on the rungs.
- Stay off the platform and top of stepladders.
- All ladders must be of proper length for the job and in good condition.
- The area around the bottom of all ladders must be free from slippery substances and tripping hazards.
- Preference is given to wooden ladders. Fibreglass ladders may be used with prior approval from
Installation Representative.

7.7 SCAFFOLDS
- Use of bamboo scaffolds shall be limited and requires prior approval of Installation Representative.
- Ladders, handrails, midrails, and toe boards are required.
- The use of Wheel scaffolds requires approval of the Installation Representative.
- Wheel scaffolds should not be relocated, occupied with worker(s).
- Wheel scaffolds in usage should be blocked.
- Do not move or slide scaffolds; scaffolds shall be disassembled and relocated.
- Tie off at top, if over two lifts high.
- All scaffolds shall be locked in or tied off at every 5 metres. Anchor or brace the running scaffold
approximately every 10 metres.
- Ladders must be used for access - never climb bracing.
- Workers must wear safety harnesses properly tied off to other structures, not tied off to the scaffold.
7.8 WELDING AND BURNING
- A "hot work" permit must be obtained before starting hot work.
- Check equipment for safe working. The Contractor shall provide dry chemical extinguishers.
- Remove combustible materials from the immediate area.
- Keep welding leads and gas hose clear of passageways.
- Special care must be taken during overhead cutting and welding operations to safeguard workers and
to prevent falling sparks from starting a fire. Warning signs must be posted at each level below the
area of any overhead welding, burning or cutting operation.
- To minimise of intermediate arcing during arc welding, the grounding lead must be connected as
close as practical to the work piece.
- Ground leads shall not be connected to plant structures /fixtures /fittings like cable trays /pipe racks.
- Use trolley or carrier to move gas cylinders.
- Do not lift or carry cylinders by handling their valves or fittings.
- Do not use cylinders as supports or as rollers.
- Turn off gas when it is not being used.
- Close the supply valves and disconnect detachable hoses from the cylinders at the end of the day.
- Keep acetylene cylinders in upright position.
- The acetylene hose should be executed with a self-extinguishing non-return valve.
- What to do in case of internal combustion of acetylene cylinder? If possible cooling down as much
and fast as possible; disposing cylinder in a river. After approximately one week, check if the fire has
extinguished. Note, this cylinder should not be used anymore!!
- Do not weld /cut /grind an enclosed vessel, drum or tank, which has contained flammable substances
unless it has been tested and certified safe to work on (filled and emtied and filled with water).
- Do not look for gas leaks with a match or naked flame. Use soapy water.
- Keep all cylinders from direct sunlight, heat, flammable materials, corrosive substances and fumes.
- When welding near flammable materials, make sure that there is no danger of their coming into
contact with flying sparks.
- Protect hoses off carrying gases against damage and crimping.
- Use oil free gloves to handle oxygen cylinders, do not lubricate protection covers or else with oil!!
- Do not take oxygen or acetylene cylinders into tanks or vessels.
- Do not allow cylinders to come into contact with apparatus or live wires.
- Use correct safety equipment, welding shield, gloves and footwear.
7.9 RIGGING, HOISTING AND CRANE
- Know the proper use of equipment e.g. chain, wire rope, tirfor etc.
- Never raise a load over people.
- Know capacities of lifting equipment and weight of loads; do not overload!
- Riding on cranes is permitted only where absolutely necessary and when adequate safety provisions
such as seats, platforms, rails, etc., are provided for passengers.
- A crane safe work permit shall be obtained from the Installation Representative for cranes having a
capacity greater than ten tons or for all cranes if the lift is a critical lift.

- Following are Critical Lifts :
* The load exceeds 80% of the rated load chart,
* Failure would cause major release of hazardous material,
* Two cranes are required to make the lift.

7.10 ELECTRICAL (POWER CENTER, TOOL, ETC.)


All service and branch circuits shall be installed in accordance with application regulations. Only
electricians shall perform electrical work.
Contractors shall provide the service ground for construction site power. Driven rods shall be 3/4 inch x
3 metre. Underground Reference Drawings are required before driving rods.
Contractors are not to operate any disconnects that are over 600 V unless equipped with full flash
protection. The Installation Representative must be contacted in all cases of disconnection. Special
permit is required for this operation.
Only authorised electricians are allowed to remove the fuses. The full flash protection, equipped with
special permit, is required if it is over 600 V.
Service cables shall be 600 V type with ground and in continuous lengths without a splice. Cables
passing through work areas shall be elevated for protection or covered and arranged to eliminate tripping
hazards.
The skin of metal buildings with electrical service shall be grounded. Extension cords shall be 3-wire
types with minimum 15 Amp service rating.
For closed and humid areas: maximum voltage for tools 50 VAC or 120 VDC in combination with a
safety transformer.

8. CONTRACTOR FACILITIES

8.1 SHEDS
Each Contractor shall provide suitable offices, change sheds and lunch sheds for own use and the use of
his Sub-Contractors. The location and duration of such sheds is at the Installation Representative.

8.2 AMENITIES
Washing, Toilets, Messing
Contractors shall provide their own amenities for their own stall and workers. This will include toilet
facilities and a Contractor must arrange any disposal services required.

8.3 HOUSEKEEPING
The basic foundation for a safe, healthy and pleasant place to work is good housekeeping.
A clean and orderly place produces a pleasant atmosphere and increases the pride and morale of workers.
On the other hand, bad housekeeping will contribute to low morale, a drop in production and furthermore
increase the accident rate.
All Contractors shall observe the following rules:
- Gangways in the working area to be kept clear at all time.
- Waste materials left under machines and pathways must be cleaned up daily.
- Never throw or scatter rubbish around the site, in buildings and working areas. All rubbish and
waste materials shall be put into bins provided.
- Return tools and equipment in their proper place after use.

- Oily rags, old paint cans and containers that have held flammable liquids which are fire hazards shall
be disposed of immediately.
- Clothing should not be left hanging on electrical appliances, switches and light.
- Nails protruding from timber and other sharp protruding objects shall be removed or
covered/protected as appropriate.
- Fire fighting equipment shall not be obstructed or relocated.
- All goods shall be properly stacked.
- Floors shall be kept free from spilled oil, paint, grease etc. All spoils should be removed.
- All damaged floor surfaces shall be reported.

8.4 COMPLETION AND CLEAN-UP


On completion of the work, each Contractor shall regularly clean up the job site, remove all equipment,
tools and excess material, etc. to the approval of the Installation Representative.
Under no circumstances shall a Contractor demobilise and remove his equipment from the site without
reporting to the Installation Representative either directly, or by telephone if the work is completed out of
normal working hours.
9. FIRE PREVENTION AND PROTECTION

9.1 FIRE PREVENTION


- Keep the work areas clean and maintain good housekeeping.
- Keep flammable materials in a designated, safe storage area free from sources of ignition.
- Unexitinguished materials (matches, cigarette butts) must be stamped out before being discarded and
should be disposed of in ashtrays or containers for the purpose.
- Do not use any defective electrical equipment or fittings.
- Disconnect electrical equipment after use or when not in use.
- All forms of welding work shall only be carried out when flammable materials within the hot work
area are properly isolated.
- Keep processes in which flammable materials are used free from sources of ignition.
- All welding units, both electric and gas, must be equipped with a fire extinguisher of an appropriate
type and size.
- Fire fighting equipment must be kept clean and ready for use.
- Fire fighting equipment locations should be marked with a signboard.
- Oily rags shall be deposited in containers and covered.
- Observe all "NO SMOKING" sign boards.
- All exits and passageways shall be kept free from obstruction.
- Exercising calamity plans on a regular base.

9.2 FIRE PROTECTION


- Smoke only in designated areas.
- Know the fire alarm signal and how to turn on an alarm.
- Know where fire extinguishers are located and how to use them.
- Store FLAMMABLES in properly labelled containers.
- Accumulation of trash, oily rags, combustible materials and similar fire hazards of any nature will
not be permitted.
- The Installation Representative shall decide what temporary fire fighting equipment is required.
Contractors shall install and maintain it to be available at all time.
- There must be a fire extinguisher, water hose, or other fire control equipment easily accessible for
each welding, cutting, burning or other such operation.
- All welding and cutting torches must be equipped with reverse flow check valve.
- All alleyways, driveways, roads, stairways, ladders and transformers shall be kept clear of material
and equipment.

- There shall be a distance of 7 m of unrestricted passage to fire
- Hydrants and fire hoses. Fire hoses shall be used only in case of fire.
- There shall be at least a 1 m radius of unrestricted passage to all portable fire extinguishers.
- Refuelling of petrol and diesel equipment shall be done only in prescribed areas and with approved
equipment. Contractors shall take all measures to minimise spills and to clean up immediately any
spills, which may occur. The Installation Representative shall approve methods for cleanup.
Refuelling equipment while the engine is running is prohibited.

10. PERMIT REGULATIONS

10.1 PERMIT REQUIREMENTS.


To start all dangerous work, a permit is required. The Contractor requests a permit via the Installation
Representative. The Contractor must give a careful and complete description of the work to be done and
which personal protection means are required. Contractor shall not work outside the job scope reviewed
when the permit has been issued.
As "dangerous works" are classified:
- The use of naked flame or spark producing activities e.g. burning, welding, grinding etc.
- The use of low flashpoint materials e.g. paints, petroleum based adhesives, petroleum based solvents
for protective finishes etc.
- Radiation hazards e.g. graphing radio of welds etc.
- Petroleum /gas hazards e.g. storage /filling from bulk petrol tanks and ditto from bulk LPG tanks etc.
- "Cut ins" to existing live process systems e.g. steam, refrigerant gas, CO2, electrical supply.
- Closed and humid areas.

10.2 FORMAT OF PERMIT


It is required that all Contractors use a standard form of "Permit to Work". A sample of which is
included as Appendix B. As a standard the "Permit to Work" shall include statements and entries to be
filled in:
- This permit should be kept demonstrable by whom it is issued in the operation described. When
safety hazards necessitate (suffocate, electrocution), than at least two operators should be involved.
- This permit is only valid for the operation, location and period of time specified on it and is subject
to any additional safety conditions stated herein.
- The location of the operation, which should be confined to a single operation or operation within an
easily controlled area (e.g. one operator welding at a set location or several operators welding within
the confines of one room where all are visible one to the other).
- The type of operation and equipment used should be specified e.g. flame cutting oxyacetylene torch.
- The duration that the permit is valid for should be clearly stated and should never exceed one week.
- The authorising person should be clearly identifiable i.e. not just an illegible signature.
- Any additional safety provisions required by the authorising person should be clearly stated e.g.
provision of a fire resistant blanket for oxyacetylene cutting in an elevated position.
- The signature, designation and company of the person accepting the "Permit to Work".

10.3 ISSUE OF PERMIT


A "Permit to Work" shall only be issued by a person authorised by the Installation Representative.
All permits shall be drawn up in duplicate on a consecutive numbering system. The duplicate copy shall
be held or registered in the Site Office and, on completion of the operation covered, the original copy
shall be returned by the Contractor, thus finalising that operation.

The authorised person issuing a "Permit to Work" shall in every instance inspect the area where the
operation is to be carried out in order to ascertain:
- Any additional safety precautions that may be required.
- That otherwise the area is safe for the operation to be carried out.
- That no other hazardous operation is being carried out within that vicinity (this should be confirmed
from the "Permit to Work" register or file in the Site Office).
A "Permit to Work" shall also be obtained for a semi-permanent work place where a hazardous operation
is being carried out e.g. site welding shop. This is the only situation where a permit may be allowed to
extend for a longer period than one week and if this extension is being permitted the area shall be
inspected on a weekly basis.
10.4 ACKNOWLEDGEMENT OF PERMIT
Acknowledgement of receipt of the "Permit to Work" shall be received in the form of the signature and
designation of the competent person accepting it on behalf of the Contractor e.g. Site Manager, Site
Agent, Senior Foreman. This signature must be placed on both copies of the "Permit to Work".
The issue of this permit does not absolve the Contractor from any contractual obligation, particularly
with reference to safety on site of his own personnel, other personnel and works in general.
Operations covered by this permit will be stopped if this permit cannot be produced for inspection by
personnel authorised by the Installation Representative at the job location

10.5 PERMIT CANCELLATION


In case of Emergency
When the site alarm indicates an emergency situation:
- All permits on-site become cancelled.
- All Contractors proceed to ASSEMBLY POINT for headcount.
- All permits must be re-issued after an emergency before work may continue.
When the work is stopped due to an unsafe condition or action observed by the Installation
Representative. A new permit must be issued after this cancellation before work may continue.
A permit to Work must be reissued for any job if there is evidence that conditions have changed
subsequent to the issuance of the original permit.
ALARM SIGNAL
ON SOUNDING OF THE ALARM SIGNAL GO TO THE NEAREST ASSEMBLY POINT AT
THE ENTRANCE GATE OF THE SITE.

11. FIRST AID - INJURIES


Contractors must supply their own First Aid facilities and /or first aid employee when the Installation
Representative considers that the number of staff /workers on site warrants it.
- In the event of accidents, all possible effort to keep onlookers from the scene must be made. The only
persons that are required in such area are those directly engaged in assisting in the emergency.
The following procedure is to be followed for all injuries:
- Report all injuries to your supervisor.
- Know the exact location where the Contractor's first aid box is kept.
- Contractor's Representative is responsible for the first aid box. Checking /replenishing of the
contents must be done regularly.
- If any injury occurs, first priority is to obtain treatment for the injured person. Depending on the
severity of injury, assist the person to the First Aid Centre for treatment, call the First Aid Centre for
assistance or arrange transport to the nearest hospital. When the injured person leaves the work site.

his Supervisor or Foreman should be advised. If the person is taken off site to hospital, he should be
accompanied by another person (preferably another employee from the same Contractor).
- Employees must not attempt to remove foreign material from eyes, except by washing with water.
- In the event of exposure to chemicals, wash the eyes and/or skin thoroughly with large quantities of
water. Continue washing for at least 15 minutes.
- If illness from inhalation occurs, move the person at once to fresh air.

12. EMERGENCY RESPONSE

12.1 FIRE /ACCIDENT REPORTING

12.2 IN CASE OF FIRE OR ACCIDENT

Installation Representative as soon as possible.
 The register of chemicals (paragraph 2.3 and 7.5) should be submitted to the relevant responsible.

12.3 EVACUATION PROCEDURE


If the general alarm signal is given
GO TO EVACUATION ASSEMBLY POINTS AT THE ENTRANCE GATE OF THE SITE.
DO NOT Linger in rooms, passageways, stairways or outside exit.
DO NOT Return to your place of work until the "ALL CLEAR" is given by the site safety officer.
