We acknowledge your application to the 2020 PLM Admission Test.

Requirements for Submission to PLM Office of the University Registrar:

One (1) printed copy of your application form with pasted 1.5 X 1.5 ID picture (colored with white
background)

Original and One (1) photocopy of the following documents:

Grade 12 school ID

PSA Birth Certificate

Grade 7-11 Form 138 (Report Card)/Form 137

For Alternative Learning System (ALS) completers and Accreditation & Equivalency (A&E) Passers:
Certificate of Completion

For Manila Residents, please bring any of the following:

Applicant's parent - Proof of Billing (i.e. electricity, water, telephone, etc.)

Parent's Income Tax Return with address

Voter’s Registration Form of Student/Parent

Two (2) pieces 1.5X1.5 ID picture (colored with white background) for the test permit

Reminders:

Applicants with incomplete requirements as enumerated above will not be entertained.

The Office of the University Registrar (OUR) will issue exam permit containing the date and time of your
exam and the instructions for the examination day.

On your examination day, present the exam permit to the examiner/proctor. No permit, no exam.
