AFFIDAVIT OF LOSS

I, ELMA VILLASICA CHERNISS, of legal age, widow and a resident of

Laylay St., Bolo Apartment 203, Estaka, Dipolog City, after having been duly sworn
to in accordance with law herein below depose and say:

That, I one of the legal heirs of Spouses Faustina Villasica married to

Salustiano Villasica, the registered owner of a parcel of land which is more
particularly described and identified as follows:

A parcel of land (Lot 458-8, of the subdivision plan (LRC) Psd-71008, being
a portion of Lot 458, Manukan, Pls-29, LRC Record No. Free Patent). situated
in the Barrio of Disakan, Municipality of Manukan, Province of Zamboanga del
Norte, covered by Transfer Certificate of Title No. T-21489 containing an area
of Eleven Thousand Seven Hundred and Sixty Three (11,763) Square Meters.
Bounded on the NE., points 3-4 by Disakan River; on the SE., points 4
to 5 by Lot 458-C of the subdivision plan; on the SW., points 5 to 1 by Lot
457, Manukan, Pls-29 and points 1 to 2 by Disakan Creek; and on the
NW., points 2 to 3 by Lot 458-A, of the subdivision plan.

THAT, my cousin Erlindo Recososa was designated custodian of the

aforesaid certificate of title for so many years;

THAT, sometime in year January, 2017 the house of my cousin in Disakan,

Manukan, Zamboanga del Norte, submerged during the unexpected rise of water
leaving most of their belongings wet and soiled;

THAT, after the flood, my cousin Erlindo Recososa discovered that important
documents kept in their cabinet were destroyed. Likewise, the above-mentioned title
was also missing.

THAT, I tried to look for the aforesaid title in my cousin’s house but the same
could no longer be found despite earnest efforts to look for the same. All my actions
proved futile.

THAT, I am executing this affidavit to attest the veracity of the foregoing

statements.
