Vendor Survey

As a valued vendor of CompanyX, we are interested in your thoughts regarding several aspects of our organization.
At CompanyX we strive to provide world class products, service and value, in that, our vendors are a part of our process and
our goals, and we desire your input as we seek to ascertain what areas we excel in and likewise, areas that might need
improvement in our interactions with our you, our vendors.
Please answer the following 10 questions by simply check-marking the box associated with your rating of CompanyX for that
question with a 1 to 5 rating system (1 = lowest level and 5 = highest level).
There is also a comment section provided if you would like to directly comment on any concerns you might have.
