    INCOMING GRADE 7

    ENROLLMENT FLOW CHART

    STEP 1
    Take the following Diagnostic Tests:
    WRITTEN EXAM: May 21 onwards
    ROOM _______________
    READING EXAM: May 22 onwards
    ROOM ________________

    STEP 2
    Secure Enrollment form at Registrar’s
    Office and present the following
    credentials:
    1. Grade 6 Report Card
    2. Birth Certificate (Photo copy)
    3. Certificate of Good Moral
    4. Brigada Certificate
    5. 1 long transparent plastic envelope

    STEP 3
    Submit Filled-out Enrollment
    form at Registrar’s Office
