residence and postal address at Padre Castillo, San Pascual, Batangas.
After having duly sworn to in accordance with law, do hereby depose and
say:

1. That I am the applicant for the delayed registration of birth of

VANESSA MAE FAELDIN NARVAEZ.
2. That she was born on October 27, 1998.
3. That she is a citizen of the Philippines
4. That the reason for the delay in registering the birth was due to
non-performance of the Municipal Civil Registrar of Gutalahc,
Zamboanga Del Norte and my work was transferred to Batangas
City.
