Advance Care Planning means having discussions


with family and friends  and/ or Substitute Decision
Maker/s whileAn advance directive is a document
that tells others what you want to happen if you
need medical care and are unable to consent or
refuse treatment.

WHAT IF I CHANGE MY MIND?

Yes, it can be changed, make sure you review your


plan regularly, and make sure that Substitute
Decision Maker is still willing and able to make
decisions for you if you cannot speak for yourself.
