DETAILS OF THIRD PARTY LOSS : A Bus bearing registration number PB-07-Y-

8185 of Express Highway, Hoshiarpur involved in this accident. Moreover, the driving person
(insured) sustained injuries in the mishap and treated at DMC Ludhiana.

CAUSE AND NATURE OF ACCIDENT : As narrated and explained in claim form, the
same is reproduced hereafter:

“On the way near site of occurrence, in an attempt to avoid the collision with people crossing the
road, in negligent way, our car became out of control and dashed behind of a bus standing on the
LHS of road causing extensive damages.”

ANY OTHER COMMENTS : The damages recommended are accidental, fresh in nature
and do coincides with cause of loss.

OBSERVATION:

On receipt of kind advises from REGIONAL OFFICE, LUDHIANA, the undersigned visited
insured’s premises at A-1 Dhabba, on Phagwara – Nawanshahar Highway with a view to
survey the damages alleged to have been caused to above captioned vehicle and in order to
assess liability of Underwriters.

The vehicle was carefully examined and necessary photographs exhibiting extent of
damages sustained by the insured’s vehicle were arranged and are enclosed for your perusal
and record. Insured delivered available documents for our verification.

The loss sustained was discussed with the insured and he narrated that above vehicle met with
an accident on 26.12.2019 at about 10:3 A.M. near Bus Adda, Behram, Distt. S.B.S. Nagar.

SPOT SURVEY:

No spot survey carried out. However, the spot photographs provided by the insured are
enclosed.

POLICE REPORT:

The incidence was also reported at police station Behram, Distt. S.B.S. Nagar and a copy of
police report in punjabi script is enclosed.

R.C. VERIFICATION:

An online R.C. verification report is enclosed.

D.L. VERIFICATION:

An online D.L. verification report is enclosed.

ASSESSMENT:

The repairer provided an estimate towards repairs and replacements, which was scrutinized in
the light of our observation and after viewing the nature and extent of damages, the quantum
of loss was ascertained on all the modes of settlement.

We noticed that it is not economical to repair the subject vehicle, therefore, the loss was
discussed on TOTAL LOSS BASIS which is considered economical.

The report got delayed due to the delay in receipt of requisite documents which have been
now provided by the insured because he was seriously injured in the mishap and treated at
DMC Ludhiana.

However, for comparison purpose, initially the assessment of loss on REPAIR BASIS is
provided hereafter:

We have been arrived at above assessment only after physical verification of above vehicle and
in our opinion, the assessment may increase after dismantling of major assemblies because the
parts wiz. Wiring harness, sensors, mountings, seats, etc. etc.. found damaged but not
demanded in the estimates provided by the repairer.

The approx., salvage value of the assessed parts is Rs. 15000.00
