This is to certify that LEMUEL G. FAELDEN, HT-3, is a regular employee of

DepEd Division of Surigao del Sur and is assigned as SHOOL HEAD of Hinipaan
Elementary School.
Under DepEd Memorandum No. 43, s. 2020 re: Guidelines on the Alternative
Work Arrangements in the Department of Education in Light of the COVID-19
Stringent Social Distancing Measures, Non-Teaching Personnel are mandated to
report to school three times a week and two days Home-Based Work for administrative
and supervisory work stuffs.
In this case thereto, this certification is issued as his QUARANTINE PASS for his
entry to the Municipality of Lingig particularly to his assigned station.
Given this March 16, 2020, at Lingig 1 District Office, Poblaion, Lingig Surigao del
Sur.
