do hereby swear and state as follows;
1. That Iam a Female adult Ugandan of sound mind with capacity
to swear this affidavit.
2. That my NSSF CARD bears a wrong name NAZZIWA
SHARON who was my former supervisor at HARRISS
INTERNATIONAL LTD but with my true passport photograph.
3. That my rightful name as per my national identity card details is
NABUKIABI MONICAH.
4. That I request NSSF to change the details of the account number
(9007300292365) to match both my rightful names as is the
picture on the same account.
5. That I depone this Affidavit conscientiously believing it to be
true to the best of my knowledge and belief.
