AFFIDAVIT

I, BRENDA S. PARAISO , of legal age, married , Filipino

and a resident of Cahilsot Village, Brgy. Calumpang General
Santos City , after being duly sworn, hereby depose and
state:

1. That I am the sister of the late PEDRO S.

SUMAGAYSAY , who was a member of Survivor’s of
Agrichemcials in Gensan (SAGING) and to negotiate
the claim with Dow Chemicals , Occidntal, Dole
Chemicals, Chiquita Companies Corporation of
Incorporation of the Philippines;

2. That this affidavit is executed in connection with

my request with Survivors of Agrichemicals in
(Gensan) Inc., to release to me whatever benefits
which is due;

3. That I hereby release and free the Survivors of

Agrichemicals in Gensan from any third party
liability which may arise for having released to
me the benefits .
