PETITION

COMES NOW petitioner CARDO DALISAY, by the undersigned

counsel, unto this Honorable Court, most respectfully avers:

1. That the petitioner who is of legal age, single, and a resident of

Brgy. 1, Laoag City, Ilocos Norte, is the maternal grandparent of
the minor, DUGONG DUTERTI;

2. That the said minor DUGONG DUTERTI, who at the time of the
filing of this petition, is only six (6) years of age is a resident of
Brgy. 2, Laoag City, Ilocos Norte. The photocopy of the
Certificate of Live Birth of the minor is herein attached as Annex
“A”;

3. That the parents of said minor, namely, Sps. VIVICO SOLO and
JOY SOLO died on January 1, 2020 due to a car accident. The
photocopies of the Certificates of Death of the deceased spouses
are herein attached as Annex “B” and Annex “C”, respectively;

4. That said minor is seized of real properties, located in Brgy. 2,

Laoag City, Ilocos Norte, of the probable value of ten million

 pesos (P10,000,000). The photocopies of the certificate of title of
said properties are herein attached as Annex “D” and Annex
“E”;

5. That due to minority of said DUGONG DUTERTI, it is necessary

and convenient that a guardian of his person and property be
appointed for the proper management and administration of said
properties;

6. That the names, ages, and residences of the relatives within the
fourth civil degree of the minor, and the person having him in his
care, are as follows, to wit:

a.) CARDO DALISAY, of legal age, and a resident of Brgy 1,

Laoag City, Ilocos Norte;

b.) BUNG GU, of legal age, and a resident of Brgy, 23, Laoag
City, Ilocos Norte;

c.) PANFY SALVA, of legal age, and a resident of Brgy. 6,

Laoag City, Ilocos Norte; and

d.) BATO DE LA ROSO, of legal age, and a resident of Brgy.

12, Laoag City, Ilocos Norte.

7. That, as above stated, CARDO DALISAY is the person having said

minor in his care, and that he possesses all the qualifications to
whom letters of guardianship should issue.

WHEREFORE, it is respectfully prayed that, upon due notice and

hearing, and upon the giving of such bond as this Honorable Court
may direct, the said petitioner, in the person of CARDO DALISAY, be
appointed guardian of the person and estate of the minor DUGONG
DUTERTI.

I, CARDO DALISAY, of legal age, Filipino, and a resident of Brgy.

2, Laoag City, Ilocos Norte, after having been duly sworn to in
accordance with law, hereby depose and state that:

1. I am the petitioner in the above-captioned case;

2. I have caused the preparation of the foregoing Petition;
3. I have read all the allegations therein which are all true and
correct of my own personal knowledge and based on authentic
records;
4. I have not commenced a similar action with any other court,
tribunal, or quasi-judicial agency;
5. Should he come to know of any, he will inform the Honorable
Court of the same within five (5) days from knowledge.
