JOINT AFFIDAVIT OF MINORITY

WE, NANCY H. REBLORA-BAÑAGA and NELSON G. BAÑAGA, married, both of

legal age, Filipinos, and residents of Tugas, Matnog, Sorsogon after having been sworn to in
accordance with law, hereby depose and state:

1. That we are the biological parents of NELVIN REBLORA BAÑAGA;

2. That NELVIN REBLORA BAÑAGA was born on March 6, 1990 at Tugas, Matnog,
Sorsogon;

3. That at the time of the conception and birth of NELVIN REBLORA BAÑAGA I Nancy
H. Reblora-Bañaga is only 22 years old and I Nelson G. Bañaga is 27, years of age;

4. That we could not have contracted marriage at that time prior to the birth of our son due
to our age, specially Nancy’s age who is then considered as still a minor;

5. That our son Nelvin was born out of wedlock hence he was registered in his Certificate of
Live Birth as NELVIN REBLORA;

6. That eventually though, we have contracted marriage on May 20, 1993 at Matnog,
Sorsogon;

7. That we are executing this affidavit to attest to the foregoing facts and for the purpose of
legitimizing the status of our son NELVIN REBLORA and to have him use the surname
“BAÑAGA”.

IN WITNESS WHEREOF, we have hereunto set our hands this 28th day of January
2020, at Irosin, Sorsogon.
