The College Admissions Center sees to it that all potential students are properly guided before during and

after their applications


for admissions. The admission process and procedure are clearly and completely presented to inform all applicants of the step by
step process so their applications are done expeditiously. The center maintains its goal to provide fast, efficient and friendly
service to all its clienteles.

Walk-In Applicant (Medicine)

1. Pay the Php600.00 admissions processing fee.


2. Undergo interview with the College of Medicine Collegiate Admissions Committee.
3. Wait for the letter of Acceptance from the Collegiate Admissions Committee of the College of Medicine.
4. Submit the rest of the required documents together with the interview results which shall serve as the final bases for the
applicant’s screening and evaluation.
5. Pay the Php5,000.00 reservation fee should the applicant pass all the criteria for screening and evaluation. The
reservation fee is not refundable. However, it shall be deducted from the tuition fees should the applicant decide to
enroll in the Institute.
6. Secure an enrollment permit.
7. Enroll.
8. Undergo physical examination.

On-Line Applicant (Medicine)

STAGE 1: PRELIMINARIES

1. Pay the Php 600.00 admissions processing fee (Filipino applicants) / $150 (Foreign applicants) via inter-bank deposit
using any of the following accounts:

A copy of the deposit slip should be e-mailed to the Collegiate Admissions Committee of the College of Medicine at
arleneola21@yahoo.com.

2. Fill-out the downloaded application form and e-mail it in PDF with the following documents:

o Copy of the Official Transcript of Records (TOR)


o Copy of National Medical Admissions Test (NMAT) Results
3. Wait for the e-mail of the Collegiate Admissions Committee of the College of Medicine for the confirmation of the
application and the schedule of the interview.

STAGE 2: SCREENING AND EVALUATION

1. Undergo an interview with the Collegiate Admissions Committee of the College of Medicine as scheduled.
2. Wait for the e-mail from the College of Medicine for the result of the application.
3. Wait for the Letter of Acceptance from the Collegiate Admissions Committee of the College of Medicine.

STAGE 3: CONFIRMATION
1. Pay the Php 5,000.00 reservation fee (or its equivalent amount in dollars) via inter-bank deposit using any of the
aforementioned DLSHSI accounts after receiving the Letter of Acceptance. The reservation fee is non-refundable.
However, it shall be deducted from the tuition fees should the applicant decide to enroll in the Institute.

Note: Dollar payments are accepted only through Chinabank.

A copy of the deposit slip should be e-mailed to the Collegiate Admissions Committee of the College of Medicine at
arleneola21@yahoo.com.

2. Submit the following documents to the Collegiate Admissions Committee of the College of Medicine on or before the
enrolment period:

a. Accomplished and signed downloadable application form


b. Authenticated National Statistics Office Birth Certificate (for Filipino Citizens)
c. Official Transcript of Records (TOR)
d. General Weighted Average (GWA) / General Point Average (GPA) Certification in the Baccalaureate Degree
e. National Medical Admission Test (NMAT) Results
f. Two (2) - page Accomplished Personal History Statement
g. Photocopy of College Diploma
h. Certification of Good Moral Character from previous School Administrator
i. Certificate of Recommendation from previous Professor
j. Certificate of Recommendation from an authority in the community
k. Five (5) copies of 2x2 color picture

Foreign Applicant (Medicine)

1. Pay the $150 admissions processing fee.


2. Submit the rest of the required documents together with the interview results which shall serve as the final bases for the
applicant’s screening and evaluation. All credentials and documents must be received on or before April 15 preceding
the start of the school year (June). This is necessary because it takes time to process the student's papers through the
Commission on Higher Education (CHED) and the Department of Foreign Affairs (DFA). Applications are processed
by the CHED on a first come-first served basis. Late applications have no assurance of being approved by the CHED
even if the student has been accepted by the medical school.
3. Undergo interview with the Collegiate Admissions Committee to determine the motivation and qualification of the
student. References and letters of recommendation are desirable.
4. Once accepted into the College of Medicine, a formal letter of acceptance shall be forwarded to the applicant.
5. Wait for the student visa (through the DLSHSI Registrar) before proceeding to Manila, otherwise, he/she has to return
to his/her homeland to receive the visa.
6. Pay the tuition fees amount to about Two Thousand Five Hundred Dollars ($2,500.00) per year. (an increase in tuition
fee of 10-20% per year is the rule).
7. Pay the $5,000 Lasallian Family Associates, Inc. Membership Fee (to be settled within the first two years of stay in the
College of Medicine).

Local Walk-In Applicant (Undergraduate)

STAGE 1: PRELIMINARIES

 Secure an application form from the College Admissions Center or Download Application Form.
 Submit the accomplished application form personally to the College Admissions Center.
 Pay the Php500.00 Allied Medical Courses Admissions Test (AMCAT) Fee at the Cashier’s Office located at the
basement of the Wang Building. Should the applicant opt to take the SPECIAL COLLEGE ADMISSION TEST FOR
ALLIED MEDICAL COURSES (SCATAMC), he/she has to pay Php700.00.
 Secure an AMCAT PERMIT from the College Admissions Center.
STAGE 2: SCREENING AND EVALUATION

 Take the AMCAT as scheduled. The applicant should be at the testing site at least 30 minutes before the examination.
The following should be brought on the day of the exam; otherwise, he/she shall not be allowed to take the exam:

1. Original entrance examination permit with 2x2 color picture holding name tag
2. Original receipt of AMCAT payment
3. One (1) valid identification card with picture
4. Two (2) Mongol pencils (no. 2 lead)
 Wait for the result of the AMCAT. Call the College Admissions Center after two (2) weeks to check if he/she can
already claim his/her official AMCAT results personally.
 Pay the admission processing fee and secure an ADMISSIONS ENVELOPE from the College Admissions Center.
 Undergo an interview with the college’s interviewers to be scheduled by the College Admissions Center.
 Submit the rest of the required documents together with the interview results which shall serve as the final bases for the
applicant’s screening and evaluation.

STAGE 1: PRELIMINARIES

 Fill-out the on-line application form and follow the instructions given.
 Submit the application and wait for a confirmation through e-mail for validation.
 Pay the Php500.00 Allied Medical Courses Admissions Test (AMCAT) Fee via inter-bank deposit using any of the
following accounts:

DEPOSIT specifying the following:

Local Transferee and Second Course Taker Applicants (Undergraduate)

STAGE 1: PRELIMINARIES

 Secure a referral slip from the College Admissions Center.


 Present the referral slip and photocopy of the Transcript of Records (TOR)/Certification of Grades to the College
Deans/Associate Deans/Collegiate Admissions Committee chairs for the preliminary interview.
 Secure an application form (only if the applicant passes the interview).
 Submit the accomplished application form to the College Admissions Center.
 Pay the Php500.00 Allied Medical Courses Admissions Test (AMCAT) Fee at the Cashier’s Office located at the
basement of the Wang Building. Should the applicant opt to take the SPECIAL COLLEGE ADMISSION TEST FOR
ALLIED MEDICAL COURSES (SCATAMC), he/she has to pay Php700.00.

Foreign Applicant (Undergraduate)

1. Secure an interview slip form the College Admissions Center.


2. Undergo initial interview with the Director for Academic Services/Concurrent Chair for Admissions and the screening
process with the Registrar.
3. Secure an application form from the College Admissions Center. Then, submit the accomplished application form.
4. Apply for the Allied Medical courses Admission Test (AMCAT) and pay the $50 foreign application fees. The
AMCAT is the DLSHSI eligibility exam for the undergraduate applicants. For the non-native English speakers, they
shall be advised to take the English Proficiency Test (EPT) first before taking the AMCAT. In the event that they fail in
the EPT, they shall be referred to the Language Services Center (LaSC) of the Institute for the English Proficiency
Program (EPP).
5. Take the eligibility exam as scheduled.
6. If eligible, secure Admissions Envelope and pay the admission processing fees.
7. Undergo interview.
8. If qualified, submit all remaining requirements.
9. Pay the Php5,000.00 reservation fees. The reservation fee is not refundable. However, it shall be deducted from the
tuition fees should the applicant decide to enroll in the institute.
10. Undergo physical examination.
11. Secure an enrollment permit.
12. Enroll.

Grounds for Denial of Admissions

 Incomplete requirements
 Failure to comply with admission policies
 Falsification of documents
 Dismissal, for whatever cause, from an applicant’s previous school
 Unavailability of slots
