AFFIDAVIT

I, JOHN BENEDICT G. BORJA, of legal age, Filipino, single, and

resident of and with postal address at Ili Norte, San Juan, La Union after
being duly sworn to in accordance with law hereby depose and state
that:

I am the guardian of minor, JAMIE C. BORJA, 11 years old, Filipino

citizen and also a resident of Ili Norte, San Juan, La Union;

Jamie’s parents, Spouses Joel M. Borja and Jeniffer C. Borja, both

OFW and presently working as Civil Engineers in Perth, Australia have no
source/s of income within the Philippines;

I am executing this Affidavit in behalf of JAMIE C. BORJA anent the

issuance of a Certification of Non-Tax Payment/Non-Filing of ITR as
required by the Bureau of Internal Revenue (BIR) relative to the
requirements for EducatIon Service Contracting (ESC) application for
JAMIE C. BORJA;

I attest to the truthulness and veracty of the foregoing.
