AFFIDAVIT OF EXPLANATION

I, CHULITO C. RUMA, of legal age, married, Filipino Citizen

and presently residing at BANGAG SOLANA CAGAYAN, after
having been duly sworn to in accordance with law do hereby depose
and say;

1. That I am a holder and in possession of Drivers License bearing

No. B0480002457 and its Original Receipt, issued by the Land
Transportation Office (LTO) that was lost on September 3,2014 copy
of the Affidavit of loss is hereto attached as ANNEXES “A” and “A-
1”, respectively;

2. On September 5, 2014 I applied for the replacement of the lost

drivers licensed issued to me by the Land Transportation Office
(LTO) at the LTO, Food Terminal Office Taguig City,

3. That one of the LTO staff instructed me to secure a clearance

from Metro Manila Development Authority (MMDA) for any traffic
violation committed prior to the replacement of my driver’s license.

4. On April 5, 2014 upon verification at the MMDA, I was

surprised when the said office informed me that I have an unsettled
violation for recklessness issued by a traffic enforcer of this
Honorable Office;

5. For the information of this Honorable Office, for my 15 years

experience behind the wheel I have not committed any violation or
reckless disregard for the safety of others nor not showing proper
concern about the possible bad results of my actions as driver;

6. That the alleged Traffic Violation issued by the apprehending

Officer was erroneous because I have no knowledge of any fact that I
have committed any violations constituting “RECKLESS
DRIVING”;
 7. That I am executing this affidavit to attest to the truth of the
foregoing and to inform the Metropolitan Manila Development
Authority (MMDA) and the Land Transportation Office (LTO) that
I have not committed the same;
