Work Immersion Agreement

This is to grant the request of the trainee referred underneath relative to his request for a work
immersion which is a requirement of the Senior High School course Inquiries, Investigations, and
Immersion.

Immersion Hours : 100 HRS
Site Assignment : The Net.Com Imus
Immediate Supervisor : Rerey Alonzo
Date of Effectivity : February 25, 2019

In view of the requirement of the said training, it is directed to consider his request, subject however to
the condition that the above-mentioned trainee shall comply accordingly and hereby accepts and agrees
to wit:

1. That he recognizes the authority of The Net.Com Imus and the area where he is assigned
and submits himself to all the rules and regulations that may be imposed in connection with
this immersion.
2. That The Net.Com Imus will not assume any responsibility whatsoever for any injury or
accident, personal or pecuniary, which may happen to him within or outside company
premises in the performance of his duties and functions during the immersion period.
3. That the student trainee renounces and waives any form of salary claim;
4. That there is no employer-employee relationship existing between the company and the
student trainee during the immersion period.
5. That the work immersion trainee, if minor, his parent or guardian binds himself to indemnify
The Net.Com Imus for damages which may be caused on any property by the work
immersion trainee intentional or neglect acts.
6. This WORK IMMERSION AGREEMENT will be in effect for the whole duration of the training.
