Padating na po ang ating “Lakbay Aral at Saya ’17-’18 (Field Trip/Educational Tour)
ngayon buwan ng Oktubre (October 26, 2017, Thursday). Inaanyayahan po namin ang lahat na
sumali at maki-lahok. Ang Pamunuan ng St. Bernadette Divine Academy ay sisiguraduhin na ito
ay magdudulot o magbibigay ng makabuluhang aral hindi lang sa ating mag-aaral pati narin sa
mga magulang. Ito na rin po ang pagkakataon na magkaruon ng “family bonding” ang mga
magulang at kanilang mga anak.
Ang mga lugar na pupuntahan at oras ay naka lista sa bandang ibaba. Hinihiling po ng
Pamunuan ng St. Bernadette Divine Academy na maaaring ang bawat istudyante ay mayroon
kasamang magulang o tagapag-alaga(chaperon) sa ating lakbay aral at saya. Ito po ay para
magkaruon ng dagdag siguridad ang ating mag-aaral sa mga lugar na pupuntahan.

FIELD TRIP FEE: Php 1,700 / per head

Requirement/s and things to bring:

 Pre-elem, Grade 1 up to Grade 6 – with chaperon(at least 1 chaperon per child)

 SBDA Bridge notebook with pencil/ballpen– for writing and note taking
 Extra clothes, money, foods, plastic bags for your trash
 Medicines for drowsiness or candies
 School I.D. and wear your P.E. uniforms
 Bring extra water
 MUST SUBMIT REPLY SLIP WITH WAIVER FORM
Take note of the ff:

 Please do not bring valuable items, SBDA and our Field Trip Facilitator is not fully
responsible for lost or damage items.
 We will NOT accept payments or “pahabol” on the day of the departure. SBDA
teachers and Administrators are facilitating the Field Trip. NO PERSONAL CHEQUE
PLEASE.
 Child and chaperon will be given a bus no. tag with corresponding seat no. on or
before October 26, 2017.
 The early you settle the payment is the possibility that you can be seated near the
driver. (First Pay, First Reserve Seat Scheme)
 Students with siblings ONLY will be seated together with their chaperon. Please no
special bus seat request (neighbors, relatives, etc. will be seated in their designated
seats)

 Maximum of two (chaperons) per child (NO “KANDONG” ALLOWED)
 Parents with babies (4 years and below) that will come have to pay half the price.
 Extra child or person must pay the full price.

participate in the mentioned activity (Educational Field Trip). I hereby authorize and consent,
ST. BERNADETTE DIVINE ACADEMY, its faculty and staff, in my behalf and in my stead, to
administer my child while participating in the school activity.
In addition, I acknowledge and consent that ST. BERNADETTE DIVINE ACADEMY
and its faculty and staff, shall not be liable for any injuries or damage that may occur to my child
while participating in a supervised activity. As a parent, I assume full responsibility.

The School is NOT FULLY LIABLE/RESPONSIBLE for:

(a) lost or damages to their personal belongings;
(this includes items such as: jewelries, electronic toys, and other expensive items)
(b) any injury or grievances inflicted to their child/children during the course of the Field
Trip;
(c) any physical, whether instant or gradual, abnormalities that transpire during the course of
the Field Trip;
