To whom it may concern,

I am Mr. Cresencio Catibog Dagos, 77 years old, widow and currently residing at Barangay
Bataan, San Juan, Batangas. I have 7 children, 4 boys and 3 girls, all of which are married
except the youngest child. All of them have works of their own. As of now I am a widow since
my wife, Carmen Lugatoc Dagos, passed away last November 10, 2008.

I have a daughter named Marissa Dagos – Jose who is currently residing and working as a nurse
in the United States of America together with her family. After a few years of living there in the
USA, I do believed that they became an immigrant already since they wanted very much to
petition me so that they can bring me too to the USA. She told me that I have to send her the
Original Birth Certificate and Marriage Contract which are all issued by the NSO. I asked my
youngest daughter, Ma. Corazon Dagos to get the necessary documents as requested from NSO
Lipa City. They are all now on my hands. After I glanced in both documents, I noticed that some
of the data stated in the Birth Certificate are different from the data stated in the Marriage
Contract.

In this connection, I would like to state the following:

1. That all of the information stated in the Birth Certificate are all true and correct. My
name is Cresencio Catibog Dagos and this is what I am using ever since;
2. That I am a legitimate resident of Barangay Bataan, San Juan, Batangas and I was born
here on June 15, 1938;
3. That my parents were FORTUNATA MALIBIRAN CATIBOG and CORNELIO
CANATUAN DAGOS, both of which had an address of Bataan, San Juan, Batangas;
4. That all of the information stated in the above 3 items are the same data stated in my
Birth Certificate with registry no. 2009-2376 and was prepared by Mrs. Adelaida L.
Gutierrez on October 23, 2009 and was received by the then Municipal Civil Registrar,
Mr. Honorio P. Pendel on November 3, 2009. The copy of my original Birth Certificate
can be obtained from the office of the NATIONAL STATISTICS OFFICE (NSO). (See
attached Birth Certificate from NSO);
5. That my name and place of residence stated in my Marriage Contract are not the same
with the data stated in the Birth Certificate. These data are not correct as stated in the
Marriage Contract since my name there is only Cresencio Catibog and the place of
residence is Barualte which is not correct. The true data are those in the Birth Certificate;
6. That after our wedding celebration, we were not able to check whatever data are in the
Marriage Contract because of excitement and can be due to lack of enough education to
know the accuracy of the data therein. I was able to finish elementary grade school while
my wife finished only grade 2. We had signed the marriage contract not knowing that the
data were incorrect;
7. That from the day we got married, we found no usage of our marriage contract, the main
reason why it is only now I was able to find out the differences;
8. That the supposed correction in our Marriage Contract can be acknowledged to be true
and correct by the two uncles of ours who were present during our wedding on February
10, 1956. They are SULPICO D. JARA who was born on March 20, 1929 and currently a
resident of Barualte, San Juan, Batangas, and DONATO L. ALMARIO who was born on
May 25, 1920 and currently residing at Bataan, San Juan, Batangas. Their Senior Citizens
ID are both attached for further references;
9. That my prayer is to have a Marriage Contract with the correct data in it especially my
name which should be CRESENCIO CATIBOG DAGOS and my place of birth should
be Bataan, San Juan, Batangas. These data can be certified true and correct by the two
persons I had mentioned in item 8 and their signatures and ID are hereby attached;
10. That I, Cresencio C. Dagos, swear to the veracity of all the information stated above to
the fullest extent of my knowledge and belief that are true in the eyes of God and to the
existing law of the land.

We, Mr. Rogelio L. Dagos and Mrs. Flordiminda M. Dagos, both of legal age, Filipino and currently
residing at 27 Periwinkle Street, BF Resort Village, Talon 2, Las Pinas City, hereby depose and state that:

1. I am the husband engaged in the private practice of profession being a Certified Public
Accountant and at the same time having a business as a Certified Pesticide Applicator while
my wife had just retired as Overseas Filipino Worker (OFW) having worked in King Faizal
Hospital and Research Center in Riyadh, Kingdom of Saudi Arabia for almost 28 years;
2. We earn an average total of One Hundred Fifty Thousand Pesos (Php 150,000.00) per month,
which is One Million Eight Hundred Thousand Pesos to Two Million Pesos annually;
3. We have two (2) children who have completed their education and both are having stable job
of their own. We have a number of nephews and nieces from our siblings;
4. Since our children are both stable in their current jobs in the Philippines, we have decided to
extend our support to our nephew, a nurse, KENNETH VERGARA DAGOS. We want our
nephew, Kenneth V. Dagos, to study further to have a brighter future as a nurse and truly
believe an Australian Education can give him better qualifications to be able to work in first
class hospitals here in the Philippines and other countries where there is a need;
5. We have decided to send and support our nephew to Charlton Brown Brisbane Australia
because their reputation is very good and they legally comply to the Australian International
student policies;
6. Our nephew, Kenneth V. Dagos has no loan since we are directly supporting him.
7. We are therefore supporting KENNETH VERGARA DAGOS education at Charlton Brown
for the duration of his Australian Student Visa.
