September 2008.

II. MATTERS TO BE INVESTIGATED:

a. The origin and cause of the fire incident
b. The suspect/perpetrators if there is any; and
c. Other matters in the occurrence of the fire incident

III. FACTS OF THE CASE:

-details of the fire incident

-physical details of the burned premises
-narration of witnesses
-other important relevant matter concerning to the
-exhaustive investigation conducted and the scene of the
 fire

IV. DISCUSSIONS
