Respected sir/madam,

First of all I want to thank Medical Institute of Tula State University and its whole
team for the contribution in my life to achieve my goal as being a doctor. Yes, I (name : Khagesh
Yadav) have graduated in PHYSICIAN - DOCTOR OF MEDICINE from TSU (Registration NO.
1912, Date of issue : july 05, 2017). Since then I have been successfully working in my country
(Nepal) as a doctor. Now I plan to do my post graduate study. For this I must need to submit
documents like diploma certificate , appendix to specialist degree and Migration Certificate to the
medical institute in order to get admission in that institute.

I have received diploma certificate, appendix to specialist degree but don’t have Migration Certificate
, which has very significant role in admission to the medical institute. What is Migration Certificate ?
The sample of migration certificate has been attached with this application.

I therefore request you to provide me with Migration Certificate and give me the opportunity
to flourish myself and the name of TSU. I will highly appreciate your concern and hope in getting
