LETTER OF UNDERTAKING

3. I understand that the admission requirements that I must submit and comply are the following:
a) Original copy of Form 138-A (Report Card of Grade 11 and Grade 12 for SHS
graduate)/Honorable Dismissal and Transcript of Records (for transferee);
b) Original copy of Certificate of Good Moral Character issued in the current year, with school
dry seal; and
c) Authenticated Birth Certificate from the Philippine Statistics Authority (PSA); PSA
Authenticated Marriage Certificate (for married female students only);

4. Among these enumerated requirements, I lack the following credentials and I promise to submit to
the Office of the University Registrar of the Technological University of the Philippines (TUP) any
time before the end of the 1st Semester, SY 2020-2021:

6. In case I fail to comply in submitting the above requirements within the period given, and if
the submitted documents will be found to be not genuine, the Technological University of the
Philippines (TUP) shall have the right to delete my name from the records of the University,
and my first semester enrolment shall be invalidated;

I fully understand the purpose, conditions and the consequences in executing this Letter of
Undertaking.
