AFFIDAVIT OF LEGITIMATION

We, spouses, Coney A. Dela Cruz and Jaymark P. Dela cruz, Filipinos, both of legal age and presently
working and residing in Apopong, General Santos City after having been sworn to in accordance with law
do hereby depose and state:

1. That we are the parents of Marco A. Dela Cruz who was born in Apopong, General Santos City on
March 8, 2016 as evidenced by the attached copy of the child’s Birth Certificate, made an integral part
hereof as Annex “A”;

2. That the birth of the aforesaid child was registered at Doctors Hospital, General Santos City;

3. That at the time of the conception of our child, there was no legal impediment for us
to contract marriage;

4. That we contracted marriage on April 20, 2018 which marriage was solemnized by Rev. Fr. Venancio
Balansag, as evidenced by the attached Marriage Contract, duly recorded and registered thereat under
Registry No. 162480, and made integral part hereof as Annex “B”;

5. That we have acknowledge our aforesaid child through his/her record of birth issued by the Local Civil
Registrar of General Santos City;

6. That I am executing this Affidavit for the registration of the legitimation of our aforesaid child and to
attest to the truth of the foregoing facts without any intent to circumvent existing laws and regulations,
and for whatever legal purpose it may serve.
