We, M/S ASHOK KUMAR AMIT KUMAR having registered office at 45A, ADHYA
SRADHYA GHAT ROAD, KOLKATA 700007 through its Authorized Signatory Mr. ASHOK
KUMAR AGARWAL, do hereby solemnly affirm and state as follows:

1. We declare that we have valid Udyog Aadhar Number (UAN) bearing UDYAM-WB-10-
0001016 and GSTN registration number NOT APPLICABLE and are eligible for
availing interest subvention under the Interest Subvention Scheme for MSMEs, 2018.

2. We also declare that we are not availing any interest subvention under any of the
Schemes of the State /Central Govt, and/ or from any Banks/Financial Institutions.

3. We confirm that, we shall abide by all the terms and conditions, other covenants and the
operational guidelines as stipulated under the Interest Subvention Schemes for MSMEs,
while availing the credit facilities from HDFC Bank Ltd.

4. We shall furnish/provide all necessary information/documents/certifications as may be

required by HDFC Bank Ltd from time to time.

This Affidavit is being furnished to HDFC Bank Ltd, for claiming the subsidy towards the
Working Capital / Term Loan facilities.

We further say that whatever stated hereinabove is true and correct to the best of our own
knowledge and records and we believe the same to be true.

3. Affidavit needs to be notarized
4. Affidavit needs to be signed by authorized signatories (as per Constitution
affixing the company seal)
5. Original needs to be submitted to respective RM and copy needs to be attached to
send for email id as provided above.

** For all other states stamp amount is Rs.200/
