Guidelines on Submission of Daily URGENT MATTERS REPORT

1. The branch clerk of court or officer-in-charge must accomplish the required


URGENT MATTERS REPORT within the day. Please see attached form
for your reference.

2. The said URGENT MATTERS REPORT shall be scanned and saved as


portable document format (PDF). This procedure is similar to the
submission of monthly report of cases to the Statistical and Reports
Division, Court Management Office, Office of the Court Administrator.

3. To ensure efficiency and uniformity, the scanned PDF file of the report
should be renamed with the following naming convention:

region.branch.court.station.reportdate

For example:

Branch 12, RTC, Zamboanga City, Zamboanga del Sur


Region 9, report dated 31 March 2020

r9.branch12.rtc.zamboangacityzamboangadelsur.31march2020

Branch 2, MTCC, Davao City, Davao del Sur


Region 11, report dated 1 April 2020

r11.branch2.mtcc.davaocitydavaodelsur.1april2020

MTC,Carmen,Agusan del Norte


Region 10, report dated 1 April 2020

r10.mtc.carmen.agusandelnorte.1april2020

1st MCTC, Compostela-New Bataan, Compostela Valley

4. After renaming your report file, you may now e-mail it to the Office of the
Deputy Court Administrator for Mindanao, Davao City, e-mail address:
odca.davao@gmail.com
