I, DONALLY G. LEYSON, of legal age, Filipino, married and a resident of Brgy.

Bubog, Numancia, Aklan, after having been duly sworn to in accordance with law,
hereby depose and state:

That I am an applicant for the Position of Court Stenographer III at the Regional
Trial Court, Branch 5, Kalibo, Aklan;

That I am willing to accept and be appointed as Court Stenographer III under a

contractual basis.
