ENDORSEMENT LETTER

To whom, it may concern,

I am writing this letter, informing you that NAME and together with her elder son NAME is
formerly residing in our residency during Enhanced Community Quarantine (ECQ) due to
Covid19 Pandemic. This letter proves that during their stay in our home we haven't observed
any symptoms and experience health issues that might be a sign of Covid19 contamination.
They also practiced necessary precautionary measures inside our house and most of all, every
time they go outside.

This information was provided based on my overall observations during their stay here, and by
reviewing their medical and health results as required by our local municipality allowing them to
travel and this gives me an idea to write a letter for them to prove that they are COVID19 free
and healthy.
