I, GADWIN E. HANDUMON, of legal age, Filipino, married, and a resident of Purok 1,

Villaflor, Oroquieta City, on oath, depose and state, that:

1. I am the General Manager of PAGLAUM MULTI-PURPOSE COOPERATIVE

that owns the business with registered name, PAGLAUM BUSINESS CENTER- Ozamiz City
located at Aguada, Ozamiz City;

2. In my capacity as General Manager, I am respectfully manifesting and certifying that

the above-named business has stopped operation with effect on July 10, 2020 because of serious
continuing losses and made worse by the Covid-19 pandemic;

3. I am executing this Affidavit of Closure for all legal intents and purposes with regard
to the said business; and

4. Affiant further sayeth naught.
