Checklist for port State Control (for Owner) Korean Register of Shipping

• For a ship navigated international voyage, this checklist should be confirmed at any time at least every 2 month.
• Master’s notification of the accident or the defects (including the repair works by the crew on board), affecting the safety
of the ship or the efficiency/completeness of its equipment on board, to the company (for notification to the Admin.), the
local agent (for notification to the PSC) and KR at the earliest opportunity is to be made, and the company’s action for
rectification at the port is to be ready.
• The Captain should hold the photo(s) and movie(if necessary) in trust. • Test hammering:
• The photo(s) for each item with ◘◘ as mandatory and with ◘ as necessary for the worst area shall be taken.
• Results# after remedial action: G ☞ Good condition, F ☞ Fair condition, P ☞ Poor condition(Repair Required)

Detainable Defects in Australian Port


* marked are most frequently detained items.
1. GMDSS Equipment *
Operation test of GMDSS(MF/HF Radio, etc.) by use of emergency power
- Fail of DSC test call (RQ àBQ); Be sure to receive BQ in Australia Port (with DC power)
- Defects on emergency power in battery room.
- Poor skill of operation by each certified officer.

2. Life boat *
- Failure to start life boat engine (primary & secondary)
- If the on-load release mechanism is incorrectly reset in accordance with the manual (be sure to lock
exactly when boat stowed & provide operating method) ;
e.g. wrong position of hyd. locking lever.
- If the on-load release mechanism, such as loose of nuts, damaged wire(or cable) is incorrectly
maintained.
- Any defects on lifeboat hull and davits (e.g. crack, hole, heavy corrosion etc. )
- Corrosion on hook hanging brackets (in and outside of boat).
- Corrosion at sheave blocks, sheave, hook ring and holding bolt/nut.
- Launching & maneuvering of life boat is not periodically carried out (every three months)

3. Fire Safety *
- Any defects of fire dampers for E/R Fan, Funnel and cargo hold & galley ventilators. e.g. failure to fully
close the damper & Heavy corrosion of inner ventilators
- Failure of fuel oil tank quick closing valves

4. Air pipe heads (Sure to open all covers and check inside) *
- Any defects of float condition ( internal corrosion, rubber gasket deteriorated/missing )
- Defects on stem for disc float (broken or missing )
- Missing of flame screen for F.O/L.O/Bilge air pipes (except WBT).

5. Emergency fire pump *


- Failure of operation of emergency fire pump including its vacuum pump
- Lag of water specific pressure when two nozzle shooting.
- Leaking on fire main on deck (check when pump running).
- Sticking on isolating valve and leaking on fire hoses.

6. Oily Water Separator *


- Failure of 15 ppm alarm & operation of solenoid valve (3 way valve)
- Periodic check & clean the filter and the connection pipe prior to outlet valve
- Confirm whether oil mixed water exists at outlet drain line of separator

7. Emergency generator *
- Failure of primary and secondary starting test (in case of having SQ test mode, auto-starting & auto ACB
on)

8. Hatch coaming plate and stays *


- Any holes or cracks (To check closely below top plates and corroded area).
- Any cracks on coaming stays and hatchway corners.
- Heavy corrosion at hatch covers ( underneath structure )
- Check leaking (crack/damage) on shell, deck, hold, Bhd, WB Tank.

9. Hatch cover cleats. *


- Missing of cleats, corrosion of cleat seats, poor condition of H/cover seal.
- Severe gaps between auto-cleats and stoppers when fully closed.

10. Securing of closing devices for access hatch and steel door *
- Missing of toggles, butterfly nuts for hatch, door and natural vents.
- Distortion(gap) of steel door and hatch cover.

11. CO2 system


- Holes or heavy corrosion on CO2 line on deck or in accommodation. *
- Poor skill of operation by officers in charge

12. Others(engine room)


- Excessive oil leaks, in particular, around generator engine & Purifier, etc. in engine room*
- M/E high pressure fuel oil pipe outer sheathing deteriorated *
- Entry of oil record book (2011 in new edition) i.e. pumping time and transfer of sludge, etc.
- Self-closers for each E/R oil tank level gauge (sight glass) defective
- Self-closing valve of sounding caps for E/R DB tanks defective.
- Portable fire-extinguisher defective

13. Others(Birdge)
- Navigation chart for sailing in the region of Australia to be kept before entering the port of
Australia.*( focus on PSC insp. in 2011). For reference, no some of AMSA PSCOs accept even though
providing the related charts by pilot or agent
- Water ingress alarm for cargo holds not working
- VDR when any miss alarm is coming* (if any, plz arrange technician)
- Compass error records
- Crew’s working and rest hour record (focus on PSC insp. in 2010)
e.g. Usually, for C/O & 1/E, one hour for overtime per day. for 2/O, 3/O, 2/E & 3/E, one or two hours per
2-3 days. In addition, actual drill & berthing to be included to overtime except officer/engineer on duty

* If major defects on hull and equipment exist, request class to take a necessary action for preventing the

(Note)
For reference, PSC officer after boarding will give last chance for the master to declare any known
deficiencies on board which have not been declared before port entry. A substantial plan with
supporting actions/documents (e.g. delivery note of spare parts, arranging technician, etc.) for
rectification of the declared deficiencies should also be ready to produce.

<Main Deficiencies imposed by USCG PSC in 2011>


1. Not enough pressure of Tow jets of water by em’cy fire pump
2. Water leakage of main fire pump
3. Malfunction of alarms and safety device of main equipment and system. Crew’s unfamiliarity.
4. Followup action not complied with ship’s SMS
5. Crew’s familiarity of equipment for fire fighting/ abandon ship drill. Failure of drills
- Failure at first try, one more chance would be given with training/prepare time. If it fail in 2nd time, it will
be detained.
- USCG recommends using lifeline during fire fighting drill and asks meaning of signal frequently.
6. Pin holes on air supply hose of SCBA. Using empty SCBA.
7. Steering gear room was filled with exhaust gas from prime mover of em’cy fire pump
8. GMDSS operator’s English ability is not enough for work
9. Load Line Marking in different from Load Line Cert.

<Recent trend of USCG PSC>


1. Detentions related to SMS failure have being increasing.
2. In some ports, especially west side of USA, inspection on GARBAGE is very strict. For instance, check the
garbage in garbage bins and it lead to deficiency if the garbage is mixed.
3. Inspection on Tankers is getting more strict because the crews’ capability is getting worse.(Inspection on
Tankers has been weak than other type of ships due to their major inspection)

- Em'cy stop test for cargo pumps from cargo manifold
