This is to authorize the bearer of this letter, MS. MARY ANNE O. SEECHUNG, to apply for
and process the necessary procedures to effect the correction of my baptismal certificate
particularly on the revision of the full name of my mother Priscila Ibasco Imperial and such other
necessary corrections before the appropriate offices or institutions. I also authorize said bearer to
do all the necessary acts, including but not limited to payment of fees and signing of documents,
in order to effect said authority. Attached herein are photocopies of my government issued
identification cards for you ready reference.
