To whom it may concern:

Here is the explanation requested in regard to my driver not wearing shoes, no first aid and
no fire extinguisher during field visit: Misc Truck Driver (Liel Trucking).

It was been committed by one of my drive , unintentionally. At the time of visit, he was
repairing the machine of the truck, surprisingly he was visited and forgot to wear shoes, and
brought out the first aid and fire extinguisher but he carried it all the time.

This matter won’t be done again. It is our responsibility to obey the workplace rules and
regulations.

We will implement the following rules and guidelines to maintain safety and to
lessen any possible problems that will come in the future.

These are what we’ll do:

1. To remind all of my employees to abide all the rules and regulations presented by
the agency.
2. To always remind them to always wear shoes, bring first aid kit and also fire
extinguisher as for safety purposes.
3. To remind them on the COVID safety precautions to avoid any problems I the
future.
4. To conduct a seminar for my employees for them to be reminded on how
important safety precautions in the working place.
