SUPPLY AGREEMENT

This Supply Agreement is entered into by and between JQ GARMENTS, with office address of
g/f LBS Bldg., Jalandoni City Proper, Iloilo City, and COACHES for the WVRAA Event 2020
collectively.

BACKGROUND:

JQ GARMENTS has been the supplier of Parade Uniforms of the Schools Division of Iloilo and
the manufacturer of the following products:

Parade Uniforms
(Jackets, Jogging Pants, Customized T-shirts and Caps).

and

the COACHES to purchase the afore-mentioned products.

Wherefore, JQ GARMENTS agreed to Supply the Jackets, Jogging Pants, Customized T-

Shirts and Caps according to the specifications and designs given and the COACHES shall
purchase the Jackets, Jogging Pants, Customized T-Shirts and Caps for the WVRAA Event
2020.

Goods shall be delivered with invoice and delivery receipts to the Supply Office, Schools
Division of Iloilo, Luna st., Lapaz, Iloilo City for their acceptance 3-5 days before the scheduled
event for distribution.

Invoices must be paid, in full, within ten (10) days after delivery.

JQ Garments will replace or repair any Parade Uniforms found to be defective.

And that all parties agree to the terms and conditions set forth above as demonstrated by their
signatures as follows:
