AFFIDAVIT ATTESTING TO ONE AND THE SAME PERSON

I, MARIA ELENA CARINO PAULINO, of legal age, Filipino, with residence

at No. 10 KH Street, Barangay West, Kamias, Quezon City after having been sworn to in
accordance with law hereby deposes and states that:

1. I was born on 3 June 1960 at Espana, Manila and that my father, Lucio Rios
Carino, and my mother, Lourdes Rosales Mores, registered me as Maria Elena Cleotilde
Mores Carino as stated in a certified true National Statistics Office (NSO) copy of my
Birth Certificate;1

2. I got married to Domingo Paulino. Since then, I used the name MARIA
ELENA CLEOTILDE C. PAULINO to reflect my married status;

3. I am known as MARIA ELENA CLEOTILDE C. PAULINO by my

relatives and neighbours;

4. In all my records, I have always used the name MARIA ELENA

CLEOTILDE C. PAULINO;

5. However, I noticed that in my Passport and Driver’s License, my name was

reflected as Ma. Elena Cleotilde C. Paulino. 2 The Department of Foreign Affairs and
Land Transportation Office contracted my name “Maria” to “Ma.” which is an acceptable
contraction according to the website www.en.wikipaedia.org;

6. Said contraction has become a common occurrence in Government Offices;

7. I am affirming to the truth of the facts as reflected in the foregoing and have
related the circumstances why there is an apparent conflict in what appears to be my
name in my Certificate of Live Birth and in some of my official records;

8. I am executing this sworn statement to apprise everyone that the name

MARIA ELENA CLEOTILDE C. PAULINO appearing in my Certificate of Live Birth
and MA. ELENA CLEOTILDE C. PAULINO, appearing in my Passport and Driver’s
License, belong to only one and the same person and for whatever legal purpose it may
serve.
