Checklist of new application for Gainful Occupation Permit

Detailed updated Curriculum Vitae (CV)

Valid Medical Certificate (General Fitness including HIV, Hepatitis B, RPR, TB and
Typhoid test (Refer to attached Medical Examination Checklist).

Translated (English or French) Education/Training/Professional/ Technical certificates.

References and certificates of Service from previous employers as to the worker’s
Competency in the trade or profession that he/she intends to follow.

Three (4) full face passport-sized photographs of all the persons mentioned on the
application form, with the full first names and surname of each individual on the
reversed side of each photo. (Instant or machine type photographs are not
acceptable)

Police certificates certifying proof of no criminal offences recorded against the

applicant, for periods of residents in those countries where the workers have resided
for a period in excess of 3 months

Copy of Valid Passport (First two pages with personal details).
