In connection with access to the official Insular Life email account under my name, I understand that:

1. The email account shall be for my sole official use, for the duration of my agency contract with Insular Life.

2. I further undertake to:

2.1. Use the email account to read and respond to company updates and announcements, and for client
communications, as appropriate;

2.2. Maintain the security of the email account by keeping my log-in credentials (i.e. password)
confidential and complying with email security features and precautions;

2.3. Not click nor access suspicious links, attachments, or other signs of phishing, malware, and the like;
instead, I should report receipt of the same to the Insular Life information security helpdesk
(infosec@insular.com.ph);

3. Use of the email account that constitutes violation of the Market Conduct Guidelines shall be subject to
the sanctions provided in the same guidelines.

4. The following shall result in immediate termination of access to and the deactivation of the email account:

4.1. Use of the account for malicious, obscene, or any other unprofessional purpose;

4.2. Non-usage of the account for 120 consecutive days. On the 90 th day of non-usage, I shall receive an
email that will serve as due notice to resume use of the email account to avoid its deactivation; and

4.3. Termination of my agent’s contract for any reason.

Upon such termination and deactivation, I shall no longer be able to access any and all messages sent,
received, or saved using the email account.

I am affixing my signature on the space provided below to signify my understanding and agreement to the
above.
